The Estimate Of Coronary Atherosclerosis Disease With Multi-noninvasive Indexes

Objective:To explore the relationship between Hs-CRP， Fbg， serumlipids,bilirubin,the composite index of serum bilirubin and serum lipid variables andcoronary atherosclerosis disease, evaluate their values in diagnosing coronaryatherosclerosis disease.Methods:This research choose129patients that do selective CAG in the secondhospital of Dalian medical university from November2010to August2012.The patientsare divided into3groups(35in contrast;49in AS;45in CHD) according to the result ofCAG. We record their age,sex,smoking,hypertension,diabetes on the day they come,onthe second day morning do the blood test of biochemical makers.Then according to theselective CAG computer their Gensini score.Use T test to compare measurementdata,use X2test to compare numeration data,use linear correlation to test the relationshipbetween those biochemical makers and Gensini score,then choose the first three muchmore related biochemical makers draw their ROC plots, compare each separatelysensitivity and specificity.Results:1、compared with the contrast,the group CHD has an higher level of Gensini score、Hs-CRP、 Fbg、 TC、 LDL-C、 LDL-C/HDL-C、 LDL-C/(TB+HDL-C)、TC/(TB+HDL-C),statistical analysis P<0.05;an lower level of HDL-C、TB、DB、IB,statistical analysis P<0.05.the group AS has a higher level of Gensini score、Hs-CRP、 LDL-C、 Fbg、 LDL-C/HDL-C、 LDL-C/(TB+HDL-C)、TC/(TB+HDL-C),statistical analysis P<0.05;a lower level of HDL-C、TB、DB、IB,statistical analysis P<0.05.The group CHD has a higher level of Gensini score、 Hs-CRP、LDL-C、Fbg、LDL-C/HDL-C、LDL-C/(TB+HDL-C)、TC/(TB+HDL-C) thanthe group AS,statistical analysis P<0.05;a lower level of HDL-C、TB、DB、IB than thegroup AS,statistical analysis P<0.05.During these three groups there is no difference ofage, sex, smoking, hypertension, diabetes,statistical analysis P>0.05.2、Hs-CRP、LDL-C、LDL-C/HDL-C、LDL-C/(TB+HDL-C)、TC/(TB+HDL-C)has a positive correlation with Gensini score,statistical analysis P<0.05；HDL-C、TB、DB、IB has a negative correlation with Gensini score,statistical analysis P<0.05.Fbg、TC has no linear correlation with Gensini score,statistical analysis P<0.05.3、According to the diagnosis with ROC plot,the AUC of Hs-CRP is0.739,theOPP is2.24，at this point the sensitivity and specificity is62.2%,76.4%;the the AUC ofLDL-C/(HDL-C+TB) is0.705, the OPP is1.28,at this point the sensitivity andspecificity is70.3%，60%；the the AUC of TC/(HDL-C+TB) is0.677,the OPP is1.75,atthis point sensitivity and specificity of is64.9%，63.6%.Conclusions:1、Hs-CRP、LDL-C、HDL-C、TB、DB、IB、LDL-C/HDL-C、LDL-C/(HDL-C+TB)、TC/(HDL-C+TB)are all take part in the coronary atherosclerosis.During themHs-CRP、LDL-C/(TB+HDL-C)、TC/(TB+HDL-C) can much better to reflect the seriousof the coronary.2、HDL-C、TB、DB、IB can protect the coronary in some extension.3、Hs-CRP、LDL-C/(HDL-C+TBIL) maybe much better to diagnose the coronaryatherosclerosis.
