Objective To explore the relationship between non fasting lipid level and coronary heart disease,coronary phenotype,number of coronary lesions,degree of coronary lesions,prognosis of patients with coronary heart disease.Methods A total of 300 patients admitted to Department of Cardiology of Affiliated Hospital of North China University of science and technology from October 2018 to December 2019 and agreed to perform coronary angiography were selected.All patients signed the letter of consent,collected the clinical data of all patients,and tested the fasting blood lipid level and non fasting blood lipid level and other biochemical indicators.According to the results of coronary angiography,they were divided into two groups,namely coronary heart disease group?239 cases?,right According to the clinical phenotype of coronary heart disease,the number of coronary artery lesions,Gensini score and mace events,61 patients in the control group were divided into different subgroups.Spss22.0 software was used to establish database and data analysis.t test,chi square test,Mann Whitney test were used U-test was used to analyze the levels of non fasting lipids and fasting lipids in different groups.Logistic regression analysis was used to analyze the correlation between non fasting lipids and coronary heart disease,coronary phenotype,number of coronary lesions,degree of coronary lesions,prognosis of patients with coronary heart disease.ROC curve analysis was used to evaluate the clinical value of non fasting lipids in the diagnosis of coronary heart disease.Results 1 The proportion of patients with smoking history and diabetes history in CHD group was higher than that in non CHD group?P<0.05?;2 The level of lntg and LDL-C in CHD group was higher than that in control group(0.57±0.43 vs 0.34±0.54 mmol.L-1),(3.05±1.15 vs 2.40±1.06 mmol.L-1),?P<0.05?.3 In the logistic regression analysis of multiple factors influencing coronary heart disease,it was found that the level of non fasting TG,non fasting LDL-C,fasting TG,fasting LDL-C increased,hypertension and diabetes were associated with coronary heart disease.The level of non fasting TG,non fasting LDL-C,fasting TG and fasting LDL-C increased,and the risk of coronary heart disease increased.4 The area under ROC curve of clinical application value of non fasting triglyceride and non fasting LDL to coronary heart disease was 0.612 and 0.662 respectively.5 The level of non fasting TC in the unstable angina group was higher than that in the stable angina group(4.89±1.36 vs 4.50±1.40 mmol.L-1),and the level of ApoA in the unstable angina group was lower than that in the unstable angina group(1.24±0.28 vs 1.38±0.27 g.L-1)?P<0.05?.6 In the logistic regression analysis of multiple factors influencing the phenotype of coronary heart disease,the decrease of ApoA level in non fasting group was correlated with the phenotype of coronary heart disease?or=0.187,95%CI=0.038-0.920??P<0.05?,the decrease of ApoA level in non fasting group was associated with the increase of the risk of unstable angina;7 The TG level in non fasting group was higher than that in sningle branch group(0.62±0.44 vs 0.46±0.41 mmol.L-1)?P<0.05?.8 In the logistic regression analysis of multiple factors affecting the number of coronary lesions,the increased level of non fasting TG was associated with the number of coronary lesions?or=2.215,95%CI=1.086-4.520??P<0.05?,and the increased level of non fasting TG increased the risk of multi vessel lesions.9 The levels of TG and LDL-C in moderate and severe lesions group were higher than those in mild lesions group(0.66±0.46 vs 0.52±0.41 mmol.L-1),(3.26±1.01 vs 2.95±1.14 mmol.L-1),and the levels of ApoA in moderate and severe lesions group were lower than those in mild lesions group(1.26±0.26 vs 1.35±0.29 g.L-1)?P<0.05?.10 In the logistic regression analysis of multiple factors affecting the number of coronary lesions,The increased level of non fasting TG was associated with the severity of coronary lesions?or=2.027,95%CI=1.087-3.708??P<0.05?,and the increased level of non fasting TG increased the severity of coronary lesions.11 There was no significant difference in the indexes of non fasting blood lipid level in patients with coronary heart disease?P<0.05?.Conclusion 1 The level of non fasting triglyceride and low-density lipoprotein cholesterol has a positive correlation with coronary heart disease.The level of non fasting triglyceride and low-density lipoprotein cholesterol increases with the risk of coronary heart disease;2 The level of non fasting apolipoprotein A decreases with the phenotype of coronary heart disease,the level of non fasting apolipoprotein a decreases with the occurrence of unstable angina;3 There was a positive correlation between the level of non fasting triglyceride and the severity of coronary lesions.1 picture;16 tables;193 references...
