Objective:To investigate the relationship of serum angiotensin Ⅱ (AngⅡ)and aldosterone (ALD)with glycosylated hemoglobin (HbAlc),age and the other biochemical index in patients with type2diabetes. Also we discusses the corelation between renin- angiotensin-aldosterone system (RAAS) and diabetic peripheral neuropathy.In addition, we observe the relationship between RAAS and yin deficiency and heat syndrome, phlegm-dampness syndrome,blood stasis syndrome with Type2diabetes patients.This study can provide certain objective reference basis for the study of TCM syndrome of type2diabetes.Methods:Sixty-three patient with type2diabetes recived measurements of AngII,AL2D, HOMA-IR, fasting blood glucose(FBG), fasting insulin, HbA1c, homeostasis model of assessment for insulin resistence index(HOMA-IR), triglyceride(TG), total cholesterol(TC), low density lipoprotein cholesterol(LDL-C), ALD and AngⅡ. Then all patients were respectively divided into two groups:hypertension group(n=21) and non- hypertension group(n=42), high-HbA1C (h-HbA1c)group(n=33) and non-high-HbA1c (non-h-HbA1c) group(n=30),elderly group(n=33) and non-elderly group(n=30),diabetic peripheral neuropathy(DPN)group and non-DPN group. All the patients diabetes dialectical points type:yin deficiency and heat syndrome group (n=27)and non-yin deficiency and heat syndrome group(n=36),phlegm-dampness syndrome group (n=34)and non-phlegm-dampness syndrome group (n=29),blood stasis syndrome group (n=18)and non-blood stasis syndrome group (n=45).The relationship among the biochemical index was analyzed.Results:There was a positive correlation of the levels of the ALD, Ang II with HbA1c.Otherwise, The levels of ALD and AngⅡ were negatively correlated with age in patients with type2diabetes. The level of Ang Ⅱ was positively correlated with FBG. ALD and AngⅡ in hypertension group and non-hypertension group was not significantly different.. However, FBG, HOMA-IR, ALD, AngⅡ in h-HbAlc group were significantly higher than in non-h-HbAlc group.Low-density lipoprotein Cholesterol(LDL-C)、 ALD and AngⅡ in elderly group were lower than in non-elderly group. Multiple regression analysis showed that Ang II was still in a significantly positive correlation with HbAlc, and ALD was in a significantly negative correlation with age. Ang Ⅱ and ALD in DPN group were significantly higher than in non-DPN group(P<0.05).DPN group was older than Non-DPN group markedly(P<0.05). Furthemore, compared non-yin deficiency and heat syndrome group, yin deficiency and heat syndrome group had higher level of FBG, ALD and Ang Ⅱ.The lever of ALD and Ang Ⅱ in the orther syndrome groups showed no statistically significant difference.Conclusions:Serum renin-angiotensin-aldosterone system are significantly associated with abnormal glucose metabolism in type2tydiabetes and ALD、Ang Ⅱ are affected by age.RAAS level of diabetic peripheral neuropathy patients was lower than without peripheral neuropathy patients.The lower level pointed out that the nerve was seriously damaged.So Ang II, ALD may become the early detection of diabetic neuropathy and the new indicators for monitoring the progression of DPN.Serum Ang II and ALD level of yin deficiency and heat syndrome with Type2diabetes patients was significantly higher than that of patients without yin deficiency and heat syndrome. Tip that serum RAAS may be used as a objective indicators of yin deficiency and heat syndrome.
