Objective: The study attempted to (1) understand the expressions of HPV,p16INK4a, Smac and Livin in cervical squamous intraepithelial lesions byutilizing the technologies of immunohitochemistry and tissue microarray and (2)determine the capability of the two methods when utilized jointly in enhancingthe detection of the diseases in quesion and prognosis. During the process of thestudy, TUNEL was also employed to detect the rate of apoptosis in cervicalsquamous intraepithelial lesions, compare the expressions of Livin and Smac,and determine the correlation between the rate of apoptosis and the developmentof cervical cancer.Method: Immunohitochemistry technique and TUNEL were employed todetect and analyze the positive rate of apoptosis and the expressions of HPV,p16INK4a, Smac and Livin in the tissue microarray prepared with the cervicaltissues obtained from53cases (9cases of normal cervix,32cases of cervicalintraepithelial neoplasia, and12cases of cervical squamous cell carcinoma), andcompare the expressions detected therein.Results:(1) The positive rates of p16INK4aand HPV16/18in the normal groupwere0.00%(0/9) and11.11%(1/9) respectively;The rates in theCIN group were78.13%(25/32) and68.75%(22/32) respectively; In the squamous cell carcinoma group, the positive rates were both91.67%(11/12). The results suggest that the differences in the ratesbetween the normal group and abnormal groups were statisticallysignificant(P<0.01). In addition, the expressions of P16INK4AandHPV16/18were closely correlated(P<0.05).(2) The positive rates of Smac and Livin in the normal group were0.00%(0/9)and22.22%(2/9) respectively;The positive ratesin the CIN group were62.50%(20/32)and90.63%(29/32)respectively;In the squamous cell carcinoma group, the positiverates both were91.67%(11/12)。The results suggest that thedifferences in the rates between the normal group and abnormalgroups were statistically significant(P<0.01). In addition, theexpression of Smac and that of Livin were closely correlated(χ2=5.3242,P<0.05).(3) The normal was0%(0/9). The positive rate of apoptosis in theCIN group was15.62%(5/32). The positive rate of apoptosis inthe squamous cell carcinoma group was66.67%(8/12). Theresults suggest that the differences in the rates between the normalgroup and abnormal groups were statistically significant(P<0.01).Conclusion:(1) The expressions of HPV16/18and p16INK4aappeared to havecorrelation with the development of squamous cell carcinoma.The correlation of the expressions suggests that they are bothinvolved in the development of squamous cell carcinoma.(2) The expressions of Smac and Livin appeared to have closecorrelation with the development of squamous cell carcinoma. The correlation of the expressions suggests that they are bothinvolved in the process of apoptosis.(3) Along with the development of squamous cell carcinoma, the rateof apoptosis increases prominently. The positive rate ofapoptosis in squamous cell carcinoma was as high as66.67%,which indicates a correlation with the development of squamouscell carcinoma.(4) It can be concluded that with the aid of tissue microarray thetechnologies of immunohitochemistry and TUNEL can becomean accurate yet economical means of detection and thus play animportant role in pathological study and diagnosis of thediseases in question.
