Objective:Study on the relationship between the serum ferritin and the early microinflammatory status of diabetic nephropathy. To provide theoretic basis for delaying the progress of early diabetic nephropathy (DN) progress by reducing the iron overload and slighting inflammation status of DN patients.Methods:To detect the serum ferritin and microinflammatory factors of the patients of DN and then analyse the relationship among the serum ferritin,24hours urine protein,β-2microglobulin, hs-CRP, TNF-a and IL-6by SPSS software.77samples of female patients with type2diabetes mellitus diagnosed according to the WHO criteria in1999were divided into3groups according to UAER levels:Group NDM, UAER less than30mg/24h; Group EDN, appear continuity micro-albuminuria state, UAER between30-300mg/24h; Group CDN, UAER continuity exceed than300mg/24h. The33control samples were come from the health women of examination center in our hospital.Results:There were no significant differences among the clinical common indicators age, DBP, HDL-C between the patients and the control group, while that of SBP, FBS, CHOL, TG, LDL-C, SU, Cr and UA is opposite.SF levels and proinflammatory molecules of microinflammatory state (hsCRP、IL-6、TNF-α) levels are significant difference betweendiabetic nephropathy patients and control subjects. SF levels is shown there are significant difference in three experimental groups comparing two by two (p<0.01). hsCRP levels in EDN group and in CDN group are significantly higher than control subjects(p<0.01), there is no significant difference between NDM group and control subjects (p>0.05), CDN group is significantly(p<0.01). IL-6levels in EDN group and in CDN group are significantly higher than control subjects (p<0.01), but there is no significant difference between NDM group and control subjects (p>0.05); CDN group is significantly higher than NDM group and EDN group (p<0.01), EDN group is significantly higher than NDM group. TNF-α levels is significantly higher in patients than in control subjects (p<0.01), And there are significant difference in three experimental groups comparing two by two (p<0.01)We investigated Pearson simple correlation between SF and UAER orβ-MG or hsCRP or IL-6or TNF-α in DN patients. The single factor analysis of the correlation was shown in CDN significant positive correlation between SF and UAER) or hsCRP(r=0.30,p=0.024; r=0.161,P=0.037), but it was shown no positive correlation between SF and β-MG or IL-6or TNF-α. Overall the sample Pearson simple correlation analysis of the correlation was shown significant positivecorrelation between SF and UAER(r=0.764, P<0.01) or β-MG(r=0.754, P<0.01) or hsCRP(r=0.779, P<0.01) or IL-6(r=0.736, P<0.01) or TNF-α(r=0.758, P<0.01).Conclusion:The elevated of the indicators hs-CRP, TNF-α, IL-6of microinflammatory status of the early stage of DN patients indicate that microinflammation status occurs in the patients with the early stage of DN. There are positive correlations between SF,24hours urine protein and β-2microglobulin level of DN patients, prompting that SF may forecast the progress of the early stage of DN; There are positive correlations between SF and microinflammatory state mediators hs-CRP, TNF-α, IL-6, prompting that SF may aggravate the early stage of DN patients through promoting the severity of the microinflammation status。...
