Objective:Fibro-osseous lesions of jaws (FOL) are a group of normal bone tissue by fibrous stroma mineralization and its derivatives substituted lesions.The most common is fibrous dysplasia (FD) and ossifying fibroma (OF).There is some overlappings in the clinical manifestations and pathological features. The pathogenesis is complex and has not yet been fully elucidated.We seek to detect RANKL,OPG gene and protein in the FD and OF lesion tissues and elucidate the role of RANKL and OPG in bone cell differentiation and metabolism,and retrospectively analyze the clinical cases.Methods:A total of forty-five patients with FD or OF lesions of jaws were treated in Oral and Maxillofacial Surgery department in the first affiliated hospital of Zhejiang University and Stomatology department in the Lishui Central Hospital from January2002to December2012,in which there were11FD cases and34OF cases. Primary cells from six cases were cultured by tissue block method and bone marrow mesenchymal stem cells were used as a control.The total RNA was extracted and the gene expressions were detected by qRT-PCR.The immunohistochemistry staining was applied to detect the subcellular location of RANKL,OPG protein in FD,OF and the normal bone of jaws. Statistical analysis of the data was performed by SPSS19.0software.Results:There existed a certain overlapping of morphological findings between FD and OF cases. qRT-PCR, and immunohistochemical staining showed that RANKL expression in FD,OF was higher than normal tissue.and there was statistical significance(P<0.05). RANKL expression in OF was higher than that in the FD and there was statistical significance(P<0.05).There was statistical signinficance of OPG expression between OF and normal tissues(P<0.05).RANKL/OPG expression in FD,OF was higher than that in normal tissue.which was statistically significant (P<0.05).ConclusionsThis study shows that FD and OF often occur in teenage.of which clinical manifestations and imaging and histology findings are similar.The correct diagnosis requires pathological combimed with clinical and radiographic findings.The serious damage of RANKL,OPG balance might be one of important mechanisms of bone destruction.The RANKL expression might induce bone resorption and lead to osteolytic lesions. The bone resorption is more active in OF than that in FD.The high expression of RANKL in FD and OF prompted RANK/RANKL/OPG signaling pathway which plays an important and indispensable role in the lesions.Correcting RANKL/OPG imbalance might be a potential therapeutic target of FD and OF.
