ObjectiveThis study was to investigate the short-term clinical efficacy andsafety of125I radioactive seed implantation in treating peritonealcarcinomatosis from Primary liver cancer guided by ultrasound.MethodsThis group of patients are9patients treated by China Japan UnionHospital of Jilin University Since October2011to October2013.Among9Patients, the ages is between40to82years old, and the median age is53years old. There were five cases of patients with pain. There were15lesions in total, including9Lymph nodes,2lesions in adrenal gland, and1gallbladder neck metastasis. Before the radioactive seed implantationdone, there were6patients whose AFP values increased. Among9patients, there were5cases of arterial chemoembolization given beforesurgery, four cases of primary liver cancer recurrence and metastasis. Allcases were confirmed through CT, MRI examination or biopsy pathology.Using real-time ultrasound guided implantation of125I seeds. All patientsunderwent CT scan2months later. Retrospective analyze Clinicalsymptoms, the circumstances of Pain Relief, the situation of local tumorcontrolled, and observe Complications. ResultsAfter2months,9cases,2obtained CR,5PR,1NC, and1PD by CTfollow-up. The total response rate was77.8%(7/9),The disease controlrate was88.9%(8/9);5patients with pain symptoms had beenalleviated, and The probability of pain relief was100%;Among6patients whose AFP values increased, there was four patients reduced tonormal levels in the first month, and two cases of postoperative secondmonth AFP value is still higher than normal. and the mean AFPdecreased by25ng/ml.All operations were successfully completed, threepatients who intra-operative puncture site bleeding,did not continuebleeding, after given symptomatic treatment. When one patient hadabdominal CT scan after two months,a walk metal grains was found inthe abdominal cavity, the sees located in the liver, and patient did notcomplain any discomfort, and no significant changes in liver functionabnormalities review. All patients had no serious complications likeradiation pneumonitis, bleeding, biliary fistula, in the surgery andpost-operation.Conclusion125I seed implantation in treating peritoneal carcinomatosis fromPrimary liver cancer under ultrasound guidance can reduce the clinicalsymptoms,improve the quality of life,effectively control the local lesion,Shorter the postoperative hospital stay,have a safe, minimally invasive, low complication rate and other advantages.
