Objective:By comparing the fiberoptic bronchoscopy brushing liquid thin-layercytology and biopsy histology, to assess the diagnostic value of fiberopticbronchoscopy brushing liquid thin-layer cytology in lung cancer, looking for areliable method for early diagnosis of lung cancer.Methods:In the period from May1st,2010to May31st,2012year,595patients,whoadmitted to the first hospital of Jilin University and were suspected lung canceron the clinical or imaging, respectively conducted the fiberoptic bronchoscopybrushing liquid thin-layer cytology examination. At the same time, all patientsaccepted the bronchial mucosa biopsy of diseased tissue or suspicious lesions.Studying on the results of cytology and histology of lung cancer diagnosis andclassification diagnosis to assess the value of fiberoptic bronchoscopy brushingliquid thin-layer cytology in the early diagnosis of lung cancer.Results:1.479patients were confirmed lung cancer by histopathology in595cases,460patients by bronchoscopy brushing TP cytology. TP cytology successfullydiagnosed417cases of479of lung cancer. The sensitivity of TP cytologymethod was87.06%, specificity62.93%, positive predictive value90.65%,negative predictive value54.07%, and the Kappa value0.471.2.479cases of lung cancer diagnosed, including223cases of squamouscell carcinoma,77cases of glandular cell carcinoma,152cases of small celllung cancer. The brushing liquid-based cytology successfully detected195cases of squamous cell carcinoma,47cases of adenocarcinoma,94cases of small celllung cancer. In the diagnosis of squamous cell carcinoma this method had thesensitivity of87.44%, the specificity of90.05%, the positive predictive value of84.05%, the negative predictive value of92.29%, and the Kappa value of0.769.The sensitivity for the diagnosis of adenocarcinoma was61.04%, the specificity90.93%, the positive predictive value50.00%, the negative predictive value94.01%, and the Kappa value0.475. The sensitivity for the diagnosis of smallcell lung cancer was61.84%, the specificity96.84%, the positive predictivevalue87.04%, the negative predictive value88.09%, and the Kappa value0.648.Conclusions:1. Fiberoptic bronchoscopy brushing liquid thin-layer cytology test couldreduce the rate of misdiagnosis to some extent.2. Fiberoptic bronchoscopy brushing liquid thin-layer cytology test hashigh ability of histological classification diagnosis, especially for squamous celllung cancer.3. Fiberoptic bronchoscopy brushing liquid thin-layer cytology test can beused as a reliable method in early diagnosis of lung cancer.
