Objective To explore the effect of the tune of Zhi on motor function of hemiplegic stroke patients with Qi deficiency and blood stasis syndrome based on the exercise training.Method90patients met the diagnostic criteria and inclusion criteria were randomly divided into the tune of Zhi group(45samples), conventional control group(45samples). The patients of conventional control group accepted general treatment and care, containing exercise training, drug, physiotherapy, nursing. Additionally, the tune of Zhi group listened the tune of Zhi after the exercise training once a day,1hour per time,5times a week for4weeks. In the pretest and posttest, all patients were assessed respectively by Fugl-Meyer assessment scale, the scale of clinical neurological deficit of stroke patient score (CNDS), the efficacy of traditional medicine symptom to assess patients’motor function, neurological deficits, the efficacy of traditional medicine symptom, and then the surface EMG was applied to record integrated EMG (IEMG) during the maximum isometric voluntary contraction (MIVC) of the knee flexors and extensor, and the co-contraction ratio(CR) was calculated by IEMG of the antagonist over the sum of IEMG of the agonist and antagonist to observe the variation of the surface EMG of patient’s muscle.Result1Baseline information(1) General information comparison result. There was no difference in the age, gender, course culture degree, hemiplegia side and the Brunnstrom stages of lower extremities between the two groups (P>0.05).(2) The indicators baseline assessment. There was no difference in the FMA score, the scale of clinical neurological deficit of stroke patient score, the efficacy of traditional medicine symptom, IEMG and CR between the two groups (P>0.05).(3) Daily record comparison results. There was no difference in the average time of watching/listening music show and additional training everyday between the two groups (P>0.05)2Fugl-Meyer assessment scaleIn the pretest, there was significant motor dysfunction in patients from the FMA score. In the posttest, there were significant improvements of the FMA score of the upper and lower extremities in both groups than pretest (P<0.01). There was no difference in the FMA score of the upper extremities between the two groups (P>0.05), but the FMA score of the lower extremities of the tune of Zhi group was higher than that of the conventional control group (P<0.05).3The scale of clinical neurological deficit of stroke patient scoreIn the pretest, there was significant nerve function defect symptoms in patients from the scale of clinical neurological deficit of stroke patient score.In the posttest, there were significant declines (P<0.01) of the neurologic deficit score in both groups than pretest, and the score of tune of Zhi group was lower than the conventional control group (P<0.05).4The efficacy of traditional medicine symptomIn the pretest, there was significant TCM syndrome in patients from the efficacy of traditional medicine symptom score. In the posttest,the tune of Zhi group was superior to conventional control group in terms of efficacy (P<0.05).5IEMG and CRIn the posttest, IEMG of recut femoris was increased during MIVC of the knee extensor in both groups than pretest (P<0.01), at the same time IEMG of biceps femoris and CR were decreased (P<0.01). In the posttest, IEMG of recut femoris was decreased during MIVC of the knee flexors in both groups than pretest (P<0.01), while IEMG of biceps femoris was increased (P<0.01) and CR were decreased (P<0.01).In the posttest, there was no significant difference between the groups on IEMG and CR (P>0.05). However, in the tune of Zhi group, IEMG of recut femoris of knee extensor was greater than that in another group about the rate of increase (P<0.01), and IEMG of biceps femoris of knee flexors was greater than that in another group about the rate of increase (P<0.01), and CR of knee flexors was greater than that in another group about the rate of decrease (P<0.01).ConclusionOn the basis of the exercise training, listening the tune of Zhi is beneficial to improve the motor function of lower extremities for the stroke patients with Qi deficiency and blood stasis syndrome, as well as the nerve system repairment and the efficacy of traditional medicine symptom. However, it didn’t work on the improvement of the motor function of upper extremities, the strength of recut femoris and biceps femoris during the MIVC and the spasticity of recut femoris.
