Objective: Explore the changes in the characteristics of left ventricularglobal strain and left ventricular Tei index in patients with hyperthyroidismalone, to evaluate the application value of ultrasound imaging in assessingthe four-dimensional strain of left ventricular function in patients withhyperthyroidism alone.Methods: Experimental group was50patients with hyperthyroidism afterthyroid function tests and clinical and laboratory diagnosis, divided into twogroups according to hyperthyroidism and hyperthyroid heart diseasediagnostic criteria: Group A, namely with hyperthyroidism alone, for theduration of less than1years, untreated patients with hyperthyroidism alone,35cases, male13cases, female22cases, mean age29.4±9.5years;GroupB,namely with hyperthyroid heart,for the diagnosis of hease in patients withhyperthyroidism,15cases, male6cases,female9cases,mean age33.9±4.3years. The control group for age, gender and the experimental group matchedhealthy volunteers, that is C group, a total of20cases,10males,10females, mean age30.1±8.5years old. The set of color Dopplerultrasonic system(GE Vivid E9) was used, equipped with M5S and4Vprobe, frequency is1.5-4.6MHz,1.5-4.0MHz, All echocardiography studieswere performed with the subjects lying in the left lateral decubitus positionconnected with electrocardiogram(ECG). Collecting all the subjects of leftventricular two-dimensional image and full volume three-dimensional image,followed by optical disc storage. Two-dimensional echocardiography: gettingparasternal left ventricular long axis view, and measurting the left ventricularend-diastolic diameter (LVDD), left ventricular end-systolic diameter(LVSD), interventricular septal thickness (IVST), left ventricular posterior wall diastolic thickness (LVPWT); in the apical four-chamber view lateralposterior septal mitral annulus tissue Doppler spectrum data and calculatedtei index were measured. Using4D automatic left ventricular cardiacfunction analysis (4D Auto LVQ) software processing the full volumedynamic image that has obtained, calculate the left ventricular end-diastolicvolume (EDV), left ventricular end-systolic volume (ESV), left ventricularejection fraction (LVEF), and the four dimensional strain parameters: leftventricular global longitudinal strain (GLS), left ventricular globalcircumferential strain (GCS), left ventricular global area strain (GAS), leftventricular global radial strain (GRS), each segment of strain parameters of17segment color encoding bovine map display.Result:1Compared three group IVST, LVPWT had no significant difference (P>0.05). GroupA and GroupC were compared: the measured values ofLVDD, LVSD,EDV, ESV,LVEF had no significant difference (P>0.05). Ingroup B: the measured values of LVDD、LVSD、EDV、ESV were comparedwith group A and group C, the difference was statistically significant(P<0.05).In group B, The values of LVDD, LVSD, EDV, ESV greater thanA group and C group mean. Group B LVEF values less than group A and group C measurements, the difference was statistically significant (P <0.05).2The left ventricular Tei index of A greater than group C, the differencewas statistically significant (P <0.05). Tei index of Group B greater thangroup A and group C, the difference was statistically significant (P <0.05).3Compared of GLS, GCS, GAS, GRS absolute value, Group A greaterthan group C, differences were statistically significant (P <0.05). Comparedof GLS, GCS, GAS, GRS absolute value,Group B less than group A andgroup C, differences had statistical significance (P <0.05).4Correlation analysis of left ventricular global strain and left ventricularejection fraction,left ventricular Tei index in patients with hyperthyroidismalone: GLS, GCS, GAS negatively correlated with left ventricular ejectionfraction, GRS positively correlated with left ventricular ejection fraction, GLS, GCS, GAS negatively correlated with left ventricular tei index, GRSpositively correlated with left ventricular Tei index.Conclusions:The structure of the left ventricle in patients with hyperthyroidism alonecomparison with the control group had no significant difference. The leftventricular systolic function of the patients with hyperthyroidism aloneenhancement, global left ventricular function decreased. Left ventricularglobal strain have relation to LVEF and left ventricular tei index in Patientswith hyperthyroidism alone. Four-dimensional ultrasound strain imagingtechniques and left ventricular Tei index can be an early, sensitive, andaccurate evaluation of left ventricular function in patients withhyperthyroidism alone.
