Objective: The purpose of this study analyzed the clinical data of inflammatory boweldisease(IBD) inpatients in First Hospital of Jilin University during a period of10year toidentify any changing trends in the clinical epidemiological feature, clinical manifestationsand treatment programs of IBD.Methods: This is a retrospective analysis of inpatients with a diagnosis of IBD in FirstHospital of Jilin University from January1,2003to December31,2012. Clinical date werecollected in our medical record room database. The study population was divided into2groups based on the year of admission (group1=IBD,2003to2007; group2=IBD,2008to2012). By the diagnosis the group1, group2are divided into four sub-groups:(group A1=UC,2003to2007; group B1=CD,2003to2007; group A2=UC,2008to2012; group B2=CD,2008to2012), comparative analysis of epidemiological trends, clinical features andtreatment programs of IBD.Results: Over the10year Period,458inpatients were registered, which contains281casesof male and female177cases; among UC374cases,214cases of male and female160cases; CD total of84cases,67males and17females. Male to female ratio of group A,group B, group A1, group A2, group B1, group B2, group1and group2are respectively1.34:1,3.94:1,1.36:1,1.32:1,5.25:1,3.54:1,1.58:1,1.59:1. During the past10years, UC, CDsex ratio did not significant change. UC patient’s sex ratio were significantly differentcompared with CD, CD patients were more common occurred in men. The mean age atdiagnosis of group A, group B, group A1, group A2, group B1, group B2, group1andgroup2are respectively44.2±18.1years,36.0±15.8years,44.5±16.8years,44.0±18.7years,42.3±14.6years,33.0±15.5years,44.2±16.5years,41.6±18.6years.During the past10years, IBD and UC have no significant change in the mean age atdiagnosis, but the mean age at diagnosis of CD gradually younger, there are significantdifferences compared patients with UC. UC patients with a history of smoking were58 cases (15.5%) and CD patients were20cases (23.8%).UC patients with a history ofdrinking were39cases(10.4%) and CD patients were6cases (7.1%). No significantdifferences in smoking history and drinking history of UC and CD inpatients. Group A ofUC patients received treatment in the gastroenterology department,department of infectious diseases, department of traditional Chinese medicine more thangroup B of CD. Group B of CD patients received treatment in surgical department,pediatrics department more than group A of UC, group2of IBD patients received treatmentin gastroenterology department, pediatrics department more than group1of IBD, insurgical department less than group1of IBD. Clinical manifestations of IBD patientsvaried, with domestic research are similar, but lower than the proportion of the varioussymptoms of domestic and foreign research. Group A of UC patients with extra-intestinalmanifestations were13cases (3.5%), and B group of CD patients with extra-intestinalmanifestations were23cases (27.4%). Extra-intestinal manifestations of CD patients havea more significant difference in UC, CD patients with extra-intestinal manifestations weremore common. Group A of UC patients with systemic manifestations:Hypoproteinemia164cases (43.9%), weight loss134cases (35.8%), anemia130cases (34.8%), fever53cases(14.2%). Group B of CD patients with systemic manifestations: Hypoproteinemia39cases(46.4%), weight loss37cases (44.1%), anemia35cases (41.7%), fever in15cases (17.9%).UC and CD patients with systemic manifestations have no significant difference,but CDpatients compared to UC have more severe systemic manifestations. IBD patients atdifferent stages, its ESR, CRP variative degrees of change.Medical and surgical treatmentof group A, group B, group A1, group A2, group B1, group B2, group1and group2arerespectively28cases (7.5%),16cases (19.1%),16cases (10.2%),12cases (5.6%), sevencases (28%),9cases (15.3%),23cases (12.6%),21cases (7.6%). UC and CD are mainlymedical treatment, during the past10years, surgical treatment of IBD patients graduallyreduced, CD patients undergoing surgical treatment are more common than the UC.Conclusion:1. During the past10years, overall, inpatients diagnosed with IBD showed an increasing trend.The increase in CD was particularly remarkable. which the first five yearsago increased gradually, after5years, maintained at a certain level.2. During the past10years, the sexual ratio between UC and CD patient have nosignificant change neither is the age ratio but the CD patients are gradually younger.3. UC inpatient’s sex ratio, age structure,intestinal manifestations,medical surgicaltreatment were significantly different compared with CD.No significant differences insmoking history,drinking history and Systemic manifestations of UC and CD inpatients.4. During the past10years, surgical treatment of IBD inpatients gradually reduced, CDinpatients undergoing surgical treatment is more common than the UC.The past five years,the application of aminosalicylate drugs, antibiotics, biologics and immunosuppressants isincreasing.
