| Objective: To prove into the effect of intravesical therapy with epirubicin orhydroxycamptothecin of preventing postoperative recurrence of the muscleinvasive bladder cancer following transurethral resection of the bladdertumor (TUR-BT).Methods: forty-three patients, who were turned out to the muscle invasivebladder cancer by the bladder neoplasm tissue biopsy pathology afterTUR-BT, were randomly divided into instantly epirubicin group (group A,n=23) and hydroxycamptothecin group (group B, n=20). The patientsaccepted intravesical instillation of epirubicin50mg (orhydroxycamptothecin40mg) dissolved into normal saline20ml within24hours after TUR-BT, opened the catheter after1hour. Then two groupsstarted routine treatment of epirubicin or hydroxycamptothecin after1weekof operation,8weeks in row. After that, the group A would accept the sametreatment1time/month×10times，a total of1year; at the same time, Bgroup would accept the same treatment1time/2weeks×8times, then1 time/month×18times, a total of2years. The tumor recurrence rates and thecomplications associated with therapy were observed between the twogroups.Result: All the patients were followed-up for thirteen to forty-four months.The one-year disease recurrence rate of epirubicin andhydroxycamptothecin was13.0%，20.0%， The two-year disease recurrencerate of epirubicin and hydroxycamptothecin was34.8%，30.0%. There wasno apparent superiority on preventing the recurrence of the bladder cancerfor two chemotherapeutics.Conclusion: Chemotherapy of intravesical epirubicin orhydroxycamptothecin instillation following TUR-BT is of good effect onthe muscle invasive bladder cancer. The epirubicin andhydroxycamptothecin showed the drug safety and clinical well tolerated.So the proper choice of treatment indications and close attention tofollow-up can immensely improve the patient’s quality of life in thepost-operation. |
