Clinical Study On Intravesical Instillation Of THP In Combination With Oral Ube Against Postoperative TURBT Recurrence Of Non-muscle Invasive Bladder Cancer

Objective: To compare the clinical effect of Pirarubicin(THP) bladder perfusion combined with oral Ubenimex(Ube) with that of pure THP perfusion on prevention of non-muscleinvasive bladder cancer(NMBC) after transurethral resection of bladder tumor(TURBt), to evaluate the postoperative recurrence rate of tumors and adverse reactions, and to provide reliable evidence for further clinical treatment of TURBt after operation.Methods: 94 cases with early bladder cancer were selected as the research objects in Qinghai University Affiliated Hospital from March to December in 2013. All the patients underwent TURBt and the pathological results after the operation all showed NMBC. According to different regimens,the patients were randomly divided into treatment group and control group. 43 cases of receiving THP bladder cancer perfusion combined with oral Ube were treatment group, while 51 cases of purely receiving THP perfusion were control group. After 1 year of follow-up, compared postoperative recurrence rate(RR) of bladder tumors and adverse reactions in patients undergone TURBt between treatment group and control group.Results: The recurrence rate of mid-risk bladder tumor was 27.8% in treatment group and 12.9% in control group, and the difference had statistical significance(P< 0.05); the recurrence rate of high-risk bladder tumor was 33.3% in control group and 25.0% in treatment group, and the difference had no statistical significance(P>0.05); the differences of bladder irritation, blood urine, liver and kidney function damage, rashes and other adverse reactions had no statistical significance(P>0.05); the occurrence rate differences of nausea, vomiting and other gastrointestinal symptoms had statistical significance(P< 0.05).Conclusions:1.The efficacy of THP bladder perfusion combined with oral Ube on prevention of the recurrence rate of mid-risk NMBC patients after TURBt is better than that of pure THP bladder perfusion. The former can well prevent the recurrence of bladder cancer after operation and has small side effects with safety and convenience.2.The long-term curative effect of THP bladder perfusion combined with oral Ube on prevention of the recurrence rate of NMBC patients after TURBt remains to be further studied.
