Three-dimensional Conformal Radiotherapy Combined With TACE VersusSurgical Resection For Hepatocellular Carcinoma With Portal Vein Tumor Thrombus: A Comparative Study

Objectives: In this study,we compared treatment effect of three-dimensionalconformal radiotherapy (3D-CRT) with surgical resection of resectablehepatocellular carcinoma (HCC) with portal vein tumor thrombus (PVTT).Methods: This study retrospectively analyzed140Patients Sufferinghepatocellular carcinoma with portal vein tumor thrombus(PVTT). Theseindividuals were divided into two groups: one group (69cases) patientsunderwent surgical resection, and the other group (71cases) patients receivedTACE combined with three-dimensional conformal radiation therapy(3D-CRT). Compared differences of survival time and complications betweenthe two groups and concluded risk factors what affect survival time andprognosis.Results: The overall median survival time was10.5months. Insurgery groupand TACE+3D-CRT group, the median survival time was10.3months and11.0months respectively. In surgery group, the survival rate of6-,12-and24–month were62.3%(43/69),42.0%(29/69),17.4%(12/69), and thesedata were66.2%(47/71),42.2%(30/71),16.9%(15/71)in TACE+3D-CRT group. The difference between the two groups was not significant (P=0.808);univariate and multivariate analysis showed that when the diameter of tumor<10cm, PVTT invasion didn’tinvolve portal vein trunk,patients who receivedTACE treatment were confirmed as good prognosis.Conclusion:There was no significant difference in survival time andprognosisbetween Surgical resection and TACE combined3D-RCT treatmentof hepatocellular carcinoma associated with portal vein tumor thrombus.prognostic factors involved size of tumor, extent of PVTT and whetherpatients underwent TACE treatment.
