Abstract/Summary:
Background: Coronary heart disease (CHD) is one of the most common heartdiseases. It is the disease which caused by coronary artery stenosis or cardiac insufficiency.It is also known as ischemic cardiomyopathy. Coronary heart disease which is the one ofthe most common diseases has become a serious harm to human life and health. Theincidence and mortality rate of CHD increase with the growth of the age, with the upwardtrend and significant gender differences, the incidence of women is lower than that of men,which suggests that it is possible that there are some connections between sex hormoneinternal environment and the incidence of CHD. In recent years, some researches onthe sex hormone and prevention coronary heart disease has been finished, butthe results were not the same, some research even draw the opposite conclusion. Now theresearches on the value of sex hormone levels in predicting the extent of coronary arterystenosis are less.Objective: To clarify the relationship between the severity of coronary arterystenosis and sex hormone level, the clinical and laboratory parameters (sex hormone, bloodglucose, blood lipids and other risk factors by multiple factors analysis) in the controlgroup and the observation one were analysed.Methods: The study enrolled252female patients who underwent coronaryangiography in the department of cardiology of first hospital affiliated to SuzhouUniversity from July2012to February2014, including control group (50persons) andobservation group (202cases, including Atherosclerosis, stable angina pectoris, unstableangina and acute coronary syndrome).Clinical baseline information were recorded in detail,including, age, height, weight, history of smoking, pertension, diabetes and drug takenhistory. Venous blood samples were taken and stored in low temperature refrigerator for examination on the first morning after admission. Then the concent rations of sexhormones, and biochemistry were determined. Statistical methods of t test, chi square testand multiple linear regression were used successively to analyse the data.Results:Clinical characteristics of the general population: compared with the controlgroup, the average age of patients in the observation group were significantly higher thanthose in control group, body mass index (BMI) had no significant difference between thetwo groups, blood lipid levels (TC,HDL, LDL and TG) were significantly different, itsP was less than0.01; in observation group, high sensitive C reactive protein (hsCRP) wassignificantly higher than that in control group (p <0.01); but there was no significantdifference in fasting blood glucose level between the two groups (p=0.2); there weresignificant difference of E2(estradiol) and E2/T (testosterone) between observation groupand the control group (p <0.05), but there was no significant difference of T level betweenthe two groups (p=0.549).The study in the observation group showed that in the whole group the E2level hadno relationship with the severity of coronary artery disease (r=-0.120, p=0.068), andthere was no obvious correlation between the T level and the severity of coronary arterylesions (r=0.118,p=0.060), while there was a significant negative correlation betweenthe E2/T ratio and the severity of coronary artery disease (Gensini score)(r=-0.328,p <0.01). Subgroup analysis suggested that early menopause (<10years or under60yearsold) female coronary artery lesion was negatively correlated with E2level; and in latemenopause or age more than60years group there was no obvious correlationbetween estrogen levels and the severity of coronary artery disease; there werenegative correlation between estrogen and TC, LDL-C, positive correlation between E2with HDL-C; compared with the normal menopau-sal group, early menopausal group(menopausal age≤45) coronary lesions were more severe.Conclusions:After the adjustment for age, dyslipidemia, and other traditional risk factorsfor coronary heart disease, there are certain correlations in postmenopau-sal femalebetween sex hormone levels and severity of coronary atherosclerosis. There was nosignificant correlation between population of estradiol, testosterone level and coronarylesion; and early menopause (<10years or younger than60years) the degree of coronaryartery disease in women was negatively correlated with the level of estradiol, latemenopause or older than60years estrogen level positively with the severity of coronary artery disease trends; compared with normal menopausal age group, early postmenopausalgroup (menopausal age <45) coronary lesions were more severe.
