Influences Of Different Coagulation Methods On Ovarian Function During Laparoscopic Ovarian Cystectomy

Objective With the development of laparoscopy,the laparoscopic ovarian cystectomyhas become the best way for removing the benign ovarian tumor in gestational agedwomen. Now it has been argued for its influences to the ovarian function with differentcoagulation methods. In this research,we compared the different influences when usingunipolar,bipolar coagulation or argon knife coagulation,which will be the theoreticalbasis for seeking better ways to protect ovarian function.Methods In our research,70patients from the second affiliated hospital of AnhuiMedical University during April to October,2012were categorized into3groupsrandomly:23cases for the unipolar coagulation group (group A),24cases for thebipolar coagulation group (group B) and23cases for the argon knife coagulation group(group C). Estra diol(E2), follicle stimulating hormone (FSH) and luteinizinghormone(LH) were tested before surgery, one month after surgery, and three monthsafter surgery, while the count of antral follicle was recorded by using transvaginal colordoppler sonography simultaneously.Results (1)Comparision of FSH,LH,E2and AFC before surgery,there was nosignificant difference(P>0.05).(2) Comparing with the levels before surgery,the E2ofgroup A one month and three months after surgery decreased significantly,while theFSH and LH levels increased at the same time,which had significant difference(P<0.05).(3)Comparing the FSH levels of group B and C before surgery, there was no significant difference (P>0.05),but comparing them after surgery one month and threemonths,the difference was significant (P<0.05).(4) The antral follicle count of allgroups were reduced one month after surgery, which had significant difference(P<0.05), and compared each of them in the three groups,the difference wassignifican(P<0.05). The antral follicle count of group A was reduced three months aftersurgery,which had significant difference (P<0.05),while group B and C did not havesuch difference(P>0.05).Conclusion Based on these results,we can see that the bipolar coagulation and argonknife are better to preserve the ovarian function during laparoscopic ovarian cystectomy.While the argon knife has certain advantages on the protection of the ovarian reserve,and is worth clinical promotion.
