Objective:To explore the clinical application value of cardiac MRI in assessment of left and right ventricular function in patients with pulmonary hypertension(PH).Methods and Materials:CMRI was performed in30PH patients(PH group) and30healthy volunteers(control group). The RV and LV function and myocardial mass were obtained by cardiac MRI in two groups.Ventricular mass index(VMI) was the ratio of RV MM and LV MM.According to the progress of illness,PH group is divided into chronic obstructive pulmonary disease (COPD) group and cor pulmonale (CP)group. The results of two groups were compared using independent-samples t-test.The results of multiple groups were compared using single factor analysis of variance(ANOVA),The results between the two groups were compared using least significant difference t test.(LSD t-test).Results:Five PH patients who were unable to perform repeated breath holding during MRI acquisition were excluded from the study. Compared with control group, The RV EDV, RV ESV and RV MM were significantly higher in PH group (P<0.05).The RV EF and RV SV were significantly lower in PH group (p<0.05).The LV EDV、SV、EF、PER and PFR were significantly lower in PH group. No significant difference of LV ESV and MMI was found between the two groups(all p>0.05).VMI in PH group was greatly higher than that in control group (p<0.05).Compared with control group, The RV EDV、RV ESV and RV MM were significantly higher in COPD and CP groups.(p<0.05).The RV EF and RV SV were significantly lower in COPD and CP groups (P<0.05).The LV SV and PFR were significantly lower in COPD group. No significant difference of LV EDV、ESV、EF、PER and MM was found between the two groups(all P>0.05).Compared with control group, The LV EDV、SV、EF PER and PFR were significantly lower in CP group. No significant difference of LV ESV and MM was found between the two groups(all P>0.05).Conclusion:When pulmonary arterial pressure continuously increases and exceeds the compensation of the right ventricle, right ventricular dilation and failure and further RV EF decrease. The left ventricular function in patients with PH are impaired too.Left cardiac diastolic function is impaired early left systolic function. CMRI could evaluate the function of RV and LV invasively and accurately, and has great valuing assessment of ventricular function in patients with PH.
