Objective:The purpose of this study was to explore these markers in the preoperative diagnosis of thyroid papillary carcinoma through the nucleic acid level detection of TFF3, GDF-15and Gal-3gene expression in papillary thyroid carcinoma, thyroid benign tumor and normal thyroid tissue, and analysis of the relationship between its expression level and the clinical pathological features of papillary thyroid carcinoma.Methods:The study subjects was randomly selected from Breast and Thyroid surgical department of Jining medical college affiliated hospital between March2012and October2013which were undergoing thyroid surgery for thyroid nodules, including30papillary thyroid carcinoma,30follicular thyroid adenoma,30nodular goiter and30normal thyroid tissue. More over, the cases of normal samples were selected from the cortical tissue around the thyroid adenoma. All the research objects were examined and approved by the Jining medical college affiliated hospital ethics committee, and written informed consent signed with the patient and family members. All patients with preoperative did not receive radiation and chemotherapy, endocrine therapy and other immune therapy. Fresh tissue samples were collected after surgical removal within0.5h, and quickly put in liquid nitrogen to preservation, and converted to-80℃refrigerator within10min to save standby. All specimens pathological types were confirmed by experienced experts in postoperative. In the collection of specimens, pay attention to collect the basic cases such as gender, age, course materials, and record the papillary thyroid carcinoma group of tumor size, degree of capsular invasion, lymph node metastasis and TNM stages. RT-PCR method was applied to detecting the expression of TFF3mRNA, GDF-15mRNA, Gal-3mRNA and reference genes β-actin mRNA in different thyroid tissues. All the data were processed by the statistical analysis software of SPSS13.0, and P<0.05was considered to be the statistical significance.Results:(1) The general information showed that sex and age of each group hadn’t show any statistical significance among these groups (P>0.05).(2) According to the results of RT-PCR, we could see that TFF3mRNA, GDF-15mRNA and Gal-3mRNA could be detected in different thyroid tissues. The content of TFF3mRNA in normal thyroid tissue group compared with the nodular goiter group had no statistical significance (Z=-1.167, P=0.243), but it is higher than follicular thyroid adenoma group (Z=-3.477, P=0.001) and papillary thyroid carcinoma group (Z=-5.101, P=0.000), which showed a statistical significance. The content of TFF3mRNA in follicular thyroid adenoma group is higher than the papillary thyroid carcinoma group (Z=-3.058, P=0.002), but lower than the nodular goiter group (Z=-3.528, P=0.000). The content of GDF-15mRNA in normal thyroid tissue group is lower than nodular goiter group (Z=-3.154, P=0.002), follicular thyroid adenoma group (Z=-2.915, P=0.004) and papillary thyroid carcinoma group (Z=-4.110, P=0.000), which showed a statistical significance. The content of GDF-15mRNA in follicular thyroid adenoma group is lower than papillary thyroid carcinoma group (Z=-2.526, P=0.012), but compared with the nodular goiter group had no statistical significance (Z=-0.614, P=0.539). The content of Gal-3mRNA in normal thyroid tissue group is lower than nodular goiter group (Z=-3.883, P=0.000), follicular thyroid adenoma group (Z=-4.749, P=0.000) and papillary thyroid carcinoma group (Z=-5.190, P=0.000), which showed a statistical significance. The content of GDF-15mRNA in follicular thyroid adenoma group is lower than papillary thyroid carcinoma group (Z=-2.713, P=0.007), but compared with the nodular goiter group had no statistical significance (Z=-0.644, P=0.520).(3) The papillary thyroid carcinoma group was grouped according to the gender, age, the tumor size, amicula infiltration degree, lymph node metastasis and TNM Stage condition. The expression levels of TFF3mRNA hadn’t showed any statistical significance among the subgroup of different gender, age, tumor size, amicula infiltration degree, lymph node metastasis, or TNM Stage (P>0.05). The expression levels of GDF-15mRNA and Gal-3mRNA in subgroup of capsular invasion, lymph node metastasis and TNM Stage Ⅲ+Ⅳ were significantly higher than in subgroup of non capsular invasion, non lymph node metastasis and TNM Stage Ⅰ+Ⅱ(P<0.05). The expression levels of GDF-15mRNA and Gal-3mRNA hadn’t showed any statistical significance among the subgroup of different gender, age or tumor size (P>0.05).Conclusion:(1) The expression levels of TFF3mRNA, GDF-15mRNA and Gal-3mRNA in papillary thyroid carcinoma compared with benign thyroid tumor had statistical significance, which showed that all these three factors above have certain reference value on nucleic acid level to identify papillary thyroid carcinoma and thyroid benign tumors.(2) The content of TFF3mRNA were lower in papillary thyroid carcinoma and follicular thyroid adenoma than those of nodular goiter and normal thyroid tissue, which showed that TFF3may play an inhibitory effect on the development of papillary thyroid tumors and follicular thyroid adenoma. The expression of GDF-15mRNA and Gal-3mRNA in papillary thyroid carcinoma, follicular thyroid adenoma and nodular goiter in significantly higher than the normal thyroid tissue, which showed that GDF-15and Gal-3may play a stimulative effect on the development of thyroid tumor.(3) The expression of GDF-15mRNA and Gal-3mRNA in subgroup of capsular invasion, lymph node metastasis and TNM Stage III+IV were significantly higher than that in subgroup of non capsular invasion, non lymph node metastasis and TNM Stage Ⅰ+Ⅱ, confirming that both of GDF-15mRNA and Gal-3mRNA are correlated to the invasion, metastasis and grade malignancy of papillary thyroid carcinoma.
