1ObjectiveThis topic combined FuFangZhenWu Granule with western medicine treatment to meet the requirements of screening patients with chronic heart failure (CHF) to observe the clinical curative effect and the level of serum IL-18, MMP-9, and the NT-proBNP MCP-1, to explore the influence of the comprehensive evaluation of compound FuFangZhenWu Granule for treatment of CHF clinical curative effect, in order to prove its possible mechanism.2Methods80patients with CHF were randomly divided into two groups,40cases.in each group.The contrast group was treated by western medicine treatment and the study group were added with FuFangZhenWuGranule on the basis of western medicine treatment for8weeks. All the patients were tested for cardiac function (NYHA), average heart rate (24h dynamic ecg), echocardiography, TCM syndrome integral, the change of the quality of life score count and serum IL-18, MMP-9, MCP-l,the NT proBNP concentration changes before and after treatment in patients.3Results(1) Comparison between before and after treatment in the study group:After treatment NYHA heart functional classes were ameliorated significantly (P<0.05), average heart rate slowed down, and echocardiogram on per guideline were improved. The symptoms and the physical signs of Traditional Chinese Medicine(TCM) were ameliorated significantly after treatment(P<0.05).Life quality were ameliorated significantly after treatment (P<0.01). IL-18MMP-9,MCP-1and NT-proBNP concentrations in serum of patients decreased significantly after treatment(P<0.01).(2) Comparison between before and after treatment in the contrast group:After treatment NYHA heart functional classes were ameliorated significantly(P<0.05), average heart rate slowed down, and echocardiogram on per guideline were improved. The symptoms and the physical signs of Traditional Chinese Medicine(TCM) were ameliorated significantly after treatment(P<0.05).IL-18MMP-9,MCP-1and NT-proBNP concentrations in serum of patients decreased significantly after treatment(P<0.01).(3) Comparison between the study group and contrast group:Standard curative effects of NYHA heart functional classes after treatment in the study group had ameliorated signi-ficantly with that of the contrast group(P<0.05). Average heart rate of the study group decreased more significantly(P<0.01).The symptoms and the physical signs integral of TCM after treatment in the study group were superior to that of the contrast group(P<0.01). The life quality of the study group were ameliorated statistical significantly compare with contrast group after treatment (P<0.01). Serum IL-18,MMP-9,MCP-1and NT-proBNP concentrations decreased significantly in the study group compare with contrast group after treatment (P<0.05). The two groups had not serious adverse reactions.ConclusionFuFangZhenWu Granule combined with western medicine in the treatment of CHF curative effect was superior to singlely western medicine, The possible mechanism of FuFangZhenWu Granule to improve heart function:(1) by improving the CHF patients with autonomic nervous function, inhibiting sympathetic nerve and activateing the vagus nerve as well as the change of hemorheology, it could reduce afterload and myocardial oxygen consumption.(2) By acting on RAAS system, it could increase the discharge and reduce water retention of sodium to reduce cardiac preload.(3) by inhibiting immune inflammation, regulating the inflammatory mediators and abnormal expression of cytokines and chemokines, it could reduce myocardial cell injury.(4) by inhibiting the degradation of collagen and the reconstruction of the matrix, it could reduce ventricular myocardial remodeling, thus delay the process of CHF.
