The Construction And Application Of House-Tree-Person Drawing Test For Clinical Evaluation System

Objective:This study the primary aim is to develop a quantitative scoring system for the H-T-P drawing test, the clinical system includes three dimensions:emotion (anxiety, depression), psychotic symptom and personality.Methods:(1) Through the literature review, summarization of clinical material and expert consulting method, preliminary constructed the items of evaluation system with four aspects including the whole feature of house-tree-person drawing test and the detail feature of the house, tree and person. The evaluation of anxiety dimension has34items, depression dimension has24items, psychotic symptoms dimension has23items and personality dimension has36items.(2) Study1:Screening evaluation items through pre-experiment.Firstly, selected185cases of anxiety disorder and216cases of depressive disorder, then ask them take the house-tree-person drawing test and Minnesota short form test. Secondly, conducted confirmatory factor analysis, internal consistency reliability analysis, raters reliability test, calibration standard correlation validity analysis based on the pre-experiment to construct the evaluation index on emotional (anxiety, depression) dimension.(3) Study2:Screening evaluation items through pre-experiment.Firstly, selected77cases of patients with schizophrenia and make the house-tree-person drawing test and Minnesota short form test. Secondly, conducted confirmatory factor analysis, internal consistency reliability analysis, raters reliability test, calibration standard correlation validity analysis based on the pre-experiment to construct the evaluation index on psychotic symptom dimension.(4) Study3:Screening evaluation items through pre-experiment.Firstly, randomly selected257cases of asymptomatic and implement the house-tree-person drawing test and Y-G personality test. Secondly, conducted confirmatory factor analysis, internal consistency reliability analysis, raters reliability test, calibration standard correlation validity analysis based on the pre-experiment to construct the evaluation index on personality dimension.(5) Study4:Preliminary application of HTP clinical evaluation system Select3clients who would like to anttend, implement the house-tree-person drawing test、 PHI test and Y-G personality test,compare the results,testing the clinical usability of HTP evaluation system.Results:(1) The evaluation of anxiety dimension has32items and contains6factors; the internal consistency reliability of anxiety is0.89; anxiety of assessment items raters Kappa coefficients are reached above0.70(p<0.01);the criterion validity is0.65, reached significant level (p<0.01).(2) The evaluation of depression dimension has23items and contains5factors, The cumulative total variance explained is79.58%; the internal consistency reliability of depression is0.82; depression subscales of raters Kappa coefficients are reached above0.70(p<0.01); the criterion validity is0.52, reached significant level (p<0.01).(3) The evaluation of psychotic symptom dimension has23items and contains5factors,the internal consistency reliability of psychotic symptom is0.90;psychotic symptoms subscales score of Kappa coefficients are reached above0.70(p<0.01);the criterion validity is0.35, reached significant level (p<0.01).(4) The evaluation of personality dimension has36items and contains6factors, the internal consistency reliability of psychotic symptom is0.88; personality subscales score of Kappa coefficients are reached above0.70(p<0.01);the criterion validity range is-0.28-0.47, reached significant level (p<0.01).Conclusion:The House-Tree-Person drawing test for clinical evaluation system has operability, reliability and validity.It can provide methods and criteria for the clinical diagnosis of house-tree-person drawing test in the clinical assessment.
