| Objective:To research the clinical efficacy of Hewei Granules to functional dyspepsia of disharmony between liver and stomach through observing the effects of symptoms, water load test and Color doppler ultrasound, serum gastric (GAS) and somatostatin (SS) compared with Domperidone and to know about the security of clinical application.Methods:80patients divided into experimental group and the control group using simple random method who were included in this study and meet the standard of the RomeⅢ of functional dyspepsia diagnostic criteria and traditional Chinese medicine (TCM) of FD syndrome of disharmony between liver and stomach diagnostic. The experimental group includes41cases who were given traditional Chinese medicine Hewei Granules of1bag/times, three times a day orally, a course of4weeks; the control group includes39cases who were given domperidone10mg/times, three times a day orally, a course of4weeks. Score the TCM symptoms respectively before and after the treatment to evaluate the improvement situation of traditional Chinese medicine symptom. Measure fasting serum gastrin (GAS) and somatostatin (SS),to evaluate its effects on gastric acid secretion, gastric bowel movement disorders and abnormal regulation of visceral sensitivity, etc. Do water load test and Color doppler ultrasound, and compare the symptoms before and after the treatment of drinking water to full water quantity to evaluate the effects on visceral sensitivity and receptive. The data were Statistical analysis with SPSS19.0software after summaried. The measurement data were compared with t-test and the categorical data were compared with Chi-square test and then concluded respectively.Results:①The traditional Chinese medicine symptoms between experimental group and control group before treatment are no significant difference (P>0.05). Two groups after the treatment of symptoms integral are difference with before treatment (P<0.05). In experimental group, epigastric pain, belching, early full remission of symptoms compared with the control group was statistically different (P<0.05) and the abdominal distension, acid reflux, heartburn, nausea and vomiting symptoms on optimal and the control group no significant difference (P>0.05). According to the results of examination, Clinical curative effect of traditional Chinese medicine show that in experimental group9cases were clinical recovery,18cases were excellent,12cases were effective,2case was invalid, and the total effective rate was95.12%and in the control group3cases were clinical recovery,11cases were excellent,16cases were effective,9cases was invalid and the total effective rate was76.92%. The cure rate, efficiency and total effective rate of the experimental geoup compared with the control group with statistically, were higher than control group (P<0.05).②Before the treatment, two groups of serum gastrin (GAS) and somatostatin (SS) levels were no significant differences (P>0.05) and were comparable. Two groups of serum GAS after treatment compared with before treatment was statistically significant, higher after treatment than before treatment (P<0.05) and two groups’ serum GAS level after treatment was no statistical difference (P>0.05). After treatment respectively compared with before treatment, two groups of serum SS was statistically significant (P<0.05), after treatment than before treatment were lower and experimental group and control group after treatment serum SS was statistically significant (P<0.05). It Suggests that the Hewei Granules can reduce the levels of serum SS effect superior than domperidone and it can improve the abnormal gastric acid secretion, gastric bowel movement disorders and abnormal visceral sensitivity of FD patients through regulating the serum GAS and SS levels.③In the water load test and Color doppler ultrasound, treatment group and control group before the treatment the TV, SPTV, SV, SPSV respectively to compare had no statistical difference (P>0.05) and comparable. Two groups after treatment compared with before treatment respectively show the TV, SPTV, SV, SPSV was significant (P<0.05) and the two groups after treatment of TV and SV is statistically significant (P<0.05) and SPTV, SPSV is no statistical difference (P>0.05). Symptoms of drinking water to full water quantity before and after therapy appeared to be on the improvement of epigastric discomfort, against the improvement of the symptoms of abdominal pain, belching, two groups are statistically significant (P<0.05) and abdominal distension and nausea symptoms is no statistical difference in two groups (P>0.05). Show Hewei Granules have an effect on gastrointestinal diastolic dysfunction and receptive. And on the improvement of the visceral hyperalgesia is superior to domperidone and the improvement of the sensitivity to mechanical stimulation such as expansion is roughly similar to domperidone.Conclusion:The Hewei Granules, which can soothe the liver, regulate the Qi Strengthen spleen harmonize the stomach, Is an effective prescription in the treatment of functional dyspepsia disharmony between liver and stomach. It can obviously improve the patient’s clinical symptoms, elevate the serum GAS, reduce the serum SS, and improve visceral sensitivity and receptive. And it’s clinical application is safety, without adverse reactions. |
