Diabetes mellitus complicated with cerebral infarction is one of the most important complications of diabetes, with the changes of the diet structure and the life rhythm of modern people, the disease incidence is increasing year by year. Western Medicine believes that the main pathological changes of the disease is atherosclerosis, its mechanism may be related to factors of hyperinsulinemia, dyslipidemia, platelet function abnormality, high blood sugar and so on. Traditional Chinese Medicine that can be classified as "Diabetes""Stroke"" Diabetes cause by stroke", this paper discusses the etiology and pathogenesis of diabetes complicated with cerebral infarction, the etiology and pathogenesis of this disease is mainly due to diabetes for a long time, dryness-heat damages the Yin fluid, lead to deficiency of Qi and Yin, blood stasis blocking meridiam, and then lead to stroke. The disease’s main performance is blood stasis, the foundation is deficiency of both Qi and Yin. Therefore, supplementing Qi and nourishing YIN and promoting blood circulation to remove obstruction in the collateral should be the usual methods to treat diabetes mellitus complicated with cerebral infarction.
