| Objective: On the inheriting and concluding the experience about the treatment of chest obstruction by applying Lu Zhizheng’s(a grandmaster of traditional Chinese medicine) method of regulating spleen and stomach, we select the damp turbidity obstructing type as a typical type for further study, assess the clinical effect of the prescription used to melt damp turbidity on chest obstruction to explore the mechanism of how this prescription works and to complete theoretical system of treatment of this method to cure obstruction.Methods:With reference to Instruction On Diagnosis And Treatment Of Chronic Stable Angina In China (2007) issued by Chinese Society of Cardiovascular Diseases of Chinese Medical Association, the standard of diagnosis of stable angina(SA) associated with labor is set. Referring to Guiding Principle On Techniques Of Clinical Research On Treating Angina Pectoris Of Coronary Heart Disease By Using Traditional Chinese Medicine And Natural Drugs(2011) issued by State Bureau of Technical Supervision(CSBTS),standard of damp turbidity obstructing type of syndrome in TCM is set to include patients scientifically. This research combines traditional Chinese medicine with western drugs and applies the method of non-parallel comparison, blocked randomization and multi-center clinical trial as well as frequency and rank-sum test to analysis obtained data. Treating plan:On the basis of using western medicine, we use TCM apozem of the prescription used to melt damp turbidity on chest obstruction. This prescription is composed of peduncle of Agastache rugosus, caulis perllae, mangnolia officinalis, toasted almond, calamus, curcuma aromatica, etc. The patients are given the herbs per100ml, twice a day (one dose a day in total).Evaluation indexes are frequency of this kind of heart attack fortnightly, change of nitrate drugs used,TCM syndrome(principle syndrome and secondary syndrome).electrocardiogram(ECG) before and after the treatment, TCH, TG, HDL-C and LDL-C,C reactive protein and changes of hepar and renal function.Result: Referring to Guiding Principle On Techniques Of Clinical Research On Treating Angina Pectoris Of Coronary Heart Disease By Using Traditional Chinese Medicine And Natural Drugs(2011) issued by CSBTS and Standard Of Assessment On The Treatment Of Angina Pectoris Of Coronary Heart Disease And ECG on the meeting whose theme was treatment of angina pectoris of coronary heart disease and arrhythmia by combining TCM and western medicine in1979,standard of treatment is set. Curative effect in group of treatment is obviously better than that in controlled group:In group of treatment, significant efficiency is56.4%efficiency is36.2%,inefficiency is7.4%; In controlled group, these rates are32.9%,49.3%and17.8%,P=0.002,<0.001, total effective rate is92.6%. ECG curative effect in group of treatment is obviously better than in controlled group: In group of treatment, significant efficiency is33.8%,efficiency is30.3%,inefficiency is35.9%;In controlled group, these rates are17.8%,32.9%and49.3%,P=0.037,<0.001, total effective rate is64.1%.Comparing group of treatment and controlled group, there is no significant difference in the reducing rate of nitroglycerin;Curative effect on TCM syndrome in group of treatment is obviously better than in controlled group, total effective rate is95.9%,symptoms such as chest distress and dizziness, have been obviously improved(P<0.05);comparing TCH, TG, HDL-C and LDL-C before and after treatment, there is a significant difference in HDL,LDL and CHO (P<0.05),no significant difference is observed in TC; no significant difference is observed in hepar and renal function.Conclusion:The prescription used to melt damp turbidity on chest obstruction can obviously improve the symptoms of SA associated with labor, ECG manifestation, level of blood fat and TCM syndrome. NO side effect and adverse reaction is observed and this result deserves attention in clinic. At the same time, this kind of TCM therapy is characterized by high safety and little side effect. And it is of great value in clinic as well as society to seek for methods and drugs in TCM which are more effective in treating angina pectoris of coronary heart disease. |
