| Purpose: Peiyu Zhang, the director of oncology who served in the China Academy of Chinese Medical Sciences, Guang’anmen Hospital, and studied under the famous Professor Hongsheng Lin, Professor Daizhao Zhang. Director Zhang has engaged in clinical work for nearly30years, and made an deep study on the clinical applications of four great classics of TCM. Moreover, director Zhang got rich experiences and original ideas in malignant tumor and relevant side effects by radiation and chemotherapy. Now, summarizing the clinical experience in cancer related anemia of director Peiyu Zhang.Methods:1. The first part:by refering to the ancient book of TCM, to learning the opinion of blood-insufficiency by the ancient physicians(most of modern physicians think the cancer related anemia belong to the category of "blood-insufficiency"). Meanwhile, by retrieving literature, o study the view and the clinical research of the modern physicians, and summarize the understanding of ancient physicians about the disease name, etiology and pathogenesis, location, syndrome differentiation and treatment and the morden medical field’s opnion and the clinical treatment about the cancer related anemia.2.The second part: through studying follow with director Peiyu Zhang on the outpatient service and ward in the past more than one year, to think deeply and summarize the understanding of the cancer related anemia about the etiology and pathogenesis, nature of disease, location and the treatment of clinical commonly used Chinese herbal medicine.Results:1. Through reading ancient book of TCM and retrieving literature, we found the cancer related anemia can be summarized in the category of "blood-insufficiency", at the same time, but need to pay attention to distinguish between "anemia" of western medicine and "blood-insufficiency" of traditional Chinese medicine (TCM).2. Each physicians think in different about the understanding of the etiology and pathogenesis. Director Zhang consider from the relationship between the qi, blood, water, fire and Tiangui. The qi, blood, water, fire alternate with each other, if one side is insufficient or too luxuriant, the rest were involved. Tiangui means kidney essence, is the congenital foundation and the postnatal foundation based in it, therefore the cancer related anemia due to the deficiency of congenital kidney essence, and affected from the dysfunction of postnatal spleen and stomach and tumor itself and the influence of chemotherapy and radiation.3. The understanding of the nature of tumor and related chemotherapy and radiation is "toxic heat" which easy to hurt yin-fluid and blood, so that deficiency of yin-fluid and blood. The nature of disease is intermingled deficiency and excess that often mixed heat and blood stasis in the early phase, and then the blood stasis is stable so that the new blood difficult to birth. At the last, blood and essence asthenia means the nature of disease is empty.4. About the location, the most physicians distinguish from the viscera. Based on it, Director Zhang emphasized the importance of spleen and stomach, and added a new thinking about a sea of blood from the human body.5. Collect effective prescription in the clinical treatment of cancer-related anemia, Guipi soup apply for the patient deficiency of heart and spleen and more tender in spleen, Zhigancao soup apply for deficiency of heart and spleen and more tender in heart, Jiaoai soup and Luguierxian soup apply for lacking of spleen yang, and Renshenyangrong soup apply for the aeipathia patient with insufficient of qi and blood. We should flexibility in the use of this Chinese herbal medicine in the clinic.Conclusion: Director Peiyu Zhang has its own unique opinion about the etiology and pathogenesis, nature of disease, location and syndrome differentiation and treatment, and has obtained the remarkable curative effect in clinic, and has provided a new thought and method for the cancer-related anemia and clinical understanding of TCM. |
