Objective to study the Change of interleukin-6(IL-6) and C-reactive protein(CRP) inPeroperative Period of Kidney stone treated respectively by Pneumatic ballisticlithotripter (APL) and dual probe ultrasonic intracorporeal lithotripter (Cyberwand).toinvestigate whether the urinary tract infection have an effect on peroperative systemicinflammatory response,whether using Cyberwand can diminish infection-associatedsystemic inflammatory response,compare with APL.Methods In all,48consecutive renal stone patients were prospectively assessed, including22patients were combined with urinary tract infection, and the other neither. Patientswith urinary tract infection were randomly divided into2groups:(APL and Cyberwand),and also the rest patients. Respectively check the plasma IL-6and CRP in3time points(24hours before procedure,24hours and72hours after procedure). In addition, measurethe pelvis pressure in patients with urinary tract infection.Result IL-6and CRP are obviously increased after procedure, and significant differenceswere observed in each group between any2time points(P<0.05). Regardless oflithotripter, IL-6and CRP are obviously variable between infection group and theno-infection group in each time poin(tP<0.05). No Significant differences were observedbetween APL and Cyberwand in IL-6and CRP, no matter infection group or no(tP>0.05).A lower pelvis pressure was noted in Cyberwand group(P<0.05),but both Cyberwandand APL groups had a low average renal pelvic pressure less than the backflow level (30 mm Hg),accumulated time of renal pelves pressure>30mm Hg in Cyberwand groupwere less than APL group(P<0.05).Conclusions IL-6and CRP are good marker which can respond surgery-induced andinfection-associated acute-phase reaction in PCNL. A lower pelvis pressure was noted inCyberwand group, however, it can not diminish infection-associated systemicinflammatory response compared with APL group.Acute-phase reaction is obviously inPCNL, when Urinary infection is severe before procedure. It is benefit for anti-infectiontherapy before procedure.
