Comparative Study The Value Of Contrast-enhanced Ultrasound By Different Injection Method In Qualitative Diagnosis Of Sentinel Lymph Nodes In Breast Cancer

Objective To compare the effectiveness of intravenous and percutaneous CEUS in thedetection and qualitative diagnosis of sentinel lymph nodes in breast cancer, to choose asuitable and effective injection method of studing breast axillary lymph node. Methods52breast cancer clinical suspects, CEUS through left ulnar vein intravenous and percutaneouswere performed in turn, observed the enhanced information of the same axillary lymphnodes,the SLNs have been recorded and marked by mark needle by observing thecontrast-enhanced pattern of sentinel lymph node in different CEUS ways, record the numberand position of enhanced SLN、enhance arrival time、duration in different inject way.Compare enhanced situation of SLN between twice CEUS, also compare withmethylthioninium chloride coloretur and histopathology. Result For52patients, wedetected57SLN by intraoperative methylthioninium chloride and histopathology.12patients were benign breast tumor, another40patients were breast cancer. By intravenousCEUS, we detected8patients with10enhanced lymph nodes, with17.54%total detectionrate、sensitiveness is21.9%、specificity is88%. By percutaneous CEUS, we detected fortypatients with enhanced lymph nodes, with41.6%detection rate for5benign cases. The restthirty five patients were in different stages of breast cancer with87.5%detection rate.Thetotal sensitivity of percutaneous CEUS was88.9%,specificity was78.9%. Conclusion(1)Compared with intravenous CEUS in qualitation of sentinel lymph node of breast cancer,percutaneous CEUS has apparent superiority.(2) Percutaneous CEUS in SLN of breast cancerwith higher detection rate, accurately qualitation, so we pretest Percutaneous CEUS hashigher value in detect、qualitation、biopsy of SLN.
