Objective Introduces a method of autologous micromorselized bone grafting at onestage to treatment infectious bone defect and fracture nonunion. Retrospective analysis andcomparison of the micromorselized bone grafting and particles bone grafting curative effect,afurther discussion and to improve the efficiency of the traditional improve the relevantfactors.Methods Retrospectively analysis in May2006to October2011, Application ofmicromorselized bone grafting and particles bone grafting method to treatment of patientswith55infectious bone defect and fracture nonunion.25cases of infectious bone defect andfracture nonunion. Infected part radical debridement, Resection inactivation organization andnecrosis tissue,application the effective external fixator fixation. Open micromorselized bonegrafting at one stage to treatment, particle diameter less than1mm. Other30cases ofinfectious bone defect and fracture nonunion. Apply the same treatment. Open particle bonegrafting at one stage to treatment,particle diameter less than5mm. When the wound coveredwith granulation tissue, have the skin graft or flap transfer operation.Results Micromorselized bone grafting of the patients after3to48months of regularfollow-up,average time of24months. All the patients are fracture healing for3to8monthsand for a average time of6months. The external fixation was removed after4to12monthsand for a average time of7months. Don’t have complications such as infection. Particle bonegrafting of the patients after8to30months of regular follow-up, average time of20months.All the patients are fracture healing for5to12months and for a average time of67months.The external fixation was removed after7to14months and for a average time of8months. Two groups in operation time basically the same premise,micromorselized bone grafting ofthe patients in hospital stays with less time an average20days, fracture healing and theexternal fixation was removed average30days in advance, Average expenses of operation toreduce13000yuan.Conclusions As long as surgical debridement thoroughly, external fixation stability,surgical operation norm and postoperative change medicine standard, autologousmicromorselized bone grafting and particle bone grafting to treatment infectious bone defectand fracture nonunion are can get good clinical curative effect. Because autologousmicromorselized bone grafting surgery can save time and cost more in hospital, so openmicromorselized bone grafting at one stage to treatment bone defect and fracture nonunion isa positive and effective method, can get ideal result.
