Objective:To detect the level of TSGF(tumor specific growth factor) in the serum beforeand after transcatheter arterial chemoembolization (TACE) of Primaryhepatocellular carcinoma (PHC) patients who have never taken therapy andcompare with AFP,CT and other traditional detection methods, discuss the value ofTSGF in diagnosis of PHC and determining the efficacy and prognostic value ofTACE.Method:The subjects are inpatients who diagnosed with PHC in our hospital fromJanuary1,2012to December31,2012.A total of86cases of PHC patients includedin the study, male accounted for73cases and female13cases; the youngest is26years old, while maximum82years old, mean age is53.5±13.2years old, medianage is51years old.All subjects performed TACE treatment in our hospital. They should detectTSGF, blood count, liver function tests before treatment,7day and1month aftertreatment,detect AFP before and1month after treatment, conduct CT scan beforeand after treatment. Through observation the level of TSGF, AFP, tumor size andlipiodol deposition shown in CT before and after treatment, the value of TSGF in thediagnosis of PHC and the relationship between the level of TSGF and therapeuticevaluation of TACE are explored. Meanwhile, by analyzing changes of TSGF beforeand after TACE treatment with AFP-negative PHC patients, a new target oftherapeutic evaluation of TACE could be provided forAFP-negative patients.Results:1、 The level of TSGF was ascendant after one week compared with thepre-operation, and the difference has statistically significant (P<0.05),while it wasdecreased after one month compared with pre-operation, and the difference hasstatistically significant (P<0.05).The level of AFP was ascendant compared one month after TACE with pre-operation, the difference has statistically significant (P<0.05).2、The change of TSGF and tumor’s longest diameter was correlation (r=0.387,p<0.001). While the change of AFP and tumor’s longest diameter was not correlation(r=0.068,p=0.531).3、TSGF was decreased significantly in CR+PR+SD group, comparingper-operation with post-operation, P<0.01, and the difference was extremelysignificant; AFP was decreased compared with post-operation, P<0.05, thedifference was significant. In PD group, the levels of AFP and TSGF have decreased,but there was no significant difference before and after treatment.4、In the group ofAFP <20ug/L and20ug/L≤AFP <400ug/L, the difference ofthe level of AFP was not significant before and after TACE therapy (P>0.05). In thegroup of AFP≥400ug/L, the level of AFP was decreased one month after TACEcompared with pre-operation. While, the levels of TSGF were significantlydecreased and there were significant differences (P<0.05) in the group of AFP <20ug/L,20ug/L≤AFP <400ug/L, AFP≥400ug/L.5、The decreased rate of TSGF has a significant difference (P<0.05) among withthe group of good, fair, poor lipiodol deposition. The better lipiodol depositionconditions, the higher the decreased rate of TSGF.Conclusion1. The level of TSGF rose one week after TACE and reduced after one month.2. The change of TSGF and tumor’s longest diameter were correlation beforeand after TACE. It could be used as an indicator of recent evaluation of the efficacyof TACE.3. In the different groups of the levels of AFP, TSGF always reduced afterTACE, there were significant differences. For AFP-negative patients, short-termeffect of TACE can be evaluated by detecting the level of TSGF.4. Joint detection of TSGF and AFP has significant value in the evaluation ofTACE short-term efficacy.
