Objective:The aim of this report was to investigate the expression of hypoxia-induciblefactor-1alpha (HIF-1α)and epithelial mesenchymal transition (EMT) related proteinfactors in urothelial carcinoma of the bladder (UCB), and their associations withpathological grading and the prognosis.Methods:Immunohistochemistry (IHC) was used to test the expression of HIF-1α,E-cadherin and Vimentin in97urothelial carcinoma of the bladder (UCB) patients.The tissue samples were confirmed as the tissue of urothelial carcinoma of thebladder. Follow-up study of postoperative life for the populations was performed,including the short and long-term complications, quality of life and1,2,5-yearsurvival rates after the surgery. IHC was used to test the expression of HIF-1α,E-cadherin and Vimentin in5bladder normal tissue as the control group.Results:All the97tissue samples from urothelial carcinoma of the bladder (UCB)patients after radical cystectomy was conformed as divide bladder by biopsy, withlow-grade carcinomas(LGC)(n=38) and high-grade carcinomas(HGC)(n=59).IHCwas used to detect the expression of low-grade carcinomas: HIF-1α:52.6%(20/38),E-cadherin:55.3%(21/38),Vimentin:36.8%(14/38).In high-grade carcinomasgroup: HIF-1α:72.9%(43/59), E-cadherin:32.2%(19/59),Vimentin:57.6%(34/59).IHC was used to detect the expression of bladder normal tissue(n=5):HIF-1α:0(0/5), E-cadherin:100%(5/5),Vimentin:0(0/5).The results of follow-up for LGC shows that among one died from kidneyfailure, and2lost follow-up. Survival at1,2and5years is rate75.0%(27/36),63.9%(23/36)and41.7%(15/36), respectively. For the HGC group, one died from heartfailure, two died from kidney failure and one of died from multiple organ failurebecause of infection, and7cases lost follow-up. Survival at1,2and5years is rate71.2%(37/52),40.4%(21/52)and13.5%(7/52).Conclusions:Hypoxia-inducible Factor-1alpha (HIF-1α), E-cadherin, and Vimentin playvital role during the progress of urothelial carcinoma of the bladder (UCB).Malignant degree for urothelial carcinoma of the bladder has a significantly positiveassociation with the expression of HIF-1α, Vimentin and a negatively withE-cadherin expression. The increase of malignancy of urothelial carcinoma of thebladder also contributes to the decrease survival rate, and can be used as prognosticindicators.
