Objective:To explore Liver resection without inflow vascular occlusion for major hepatectomy Methods: A retrospective analysis of2010January to2013December in our hospital treated by operation and768cases of malignant tumor of liver confirmed by pathology, and compared the data of the tumor size, vascular occlusion method, operation time, operation hemorrhage, liver function before operation, postoperative complications. Results:Three groups of patients with preoperative serum ALB, ALT, TB, HB no significant difference (P>0.05). After ALB, ALT, TB, HB in Pringle and anatomic occlusion, no significant difference of blood occlusion group were (P <0.05). Postoperative ALT in Pringle and anatomic occlusion, occlusion two groups had no difference (P <0.05); TB in Pringle, anatomic occlusion, no occlusion three groups have significant differences (P <0.05); HB in Pringle, anatomic occlusion, no occlusion three groups have differences sex (P <0.05); operation bleeding volume: Pringle, anatomic occlusion occlusion, no significant difference between the three groups have (P <0.05); operation time: Pringle, anatomic occlusion, no occlusion had no significant difference (P>0.05). Pringle, anatomic occlusion, no occlusion complications were33.4%,26.3%,29.2%, P>0.05, not statistically significant, Pringle group in postoperative bile leakage, hepatic insufficiency, the incidence of postoperative hepatic failure rates were6.7%,3.3%,3.3%higher than the other two groups. Conclusions:Liver resection without inflow vascular occlusion does not increase the operation time, postoperative liver function Pringle method is fast, the amount of bleeding compared with Pringle method and anatomic occlusion in slightly more, patients with hepatocellular carcinoma resection is feasible.
