Objective:1, To investigate the prevalence of childhood trauma among patientswith schizophrenia.2, To evaluate the impact of childhood trauma on age of onset, social functionbefore frst hospitalization, causes, pattern of take medicine, social support system.3, To evaluate the impact of childhood trauma on symptoms, cognitive functionand the effect of treatment in acute phase of schizophrenia.Method:120patients formed the study group. Patients were diagnosed for schizophreniaat a consensus meeting incorporating clinical, and Structured Clinical Interview forDSM-IV (SCID). We evaluated the age of onset, the psychopathology at admission,hospitalizations, pattern of take medicine, social support system. We assessed thepresence of childhood abuse and neglect by using Childhood Abuse Questionnaire(CAQ), the symptome by using Positive and Negative syndrome Scale (PANSS), theauditory hallucination by using Hoffman Hallucination, and the social support systemby using Personal and Social Performance scale (PSP) after remission at least6weeks.Results:1, One hundred patients were eligible. Frequencies of childhood sexual abuse(CSA), physical abuse (CPA), emotional abuse (CEA), physical neglect andemotional neglect (CEN) were reported by25.0%(25patients),15.0%(15patients), 15.0%(15patients),41.0%(41patients) and33.0%(33patients) of the patients withschizophrenia respectively.2, We found no significant difference in age of onset, age of first hospitalization,psychopathology at admission, hospitalizations, incentive of hospitalization, suicidalattempts, Learning difficulties, Social contact difficulties in childhood between theabused patients and the the non-abused patients in the childhood. And in the twogroups there did not showed any statistically significant differences in PANSS scaletotal score,each subscales score, digit symbol test, digit span test, word fluency testsand treatment related situation. The abused patients had worse subjective socialsupport(17.62±4.44:19.85±4.89,P<0.05).3, The patients with history of CEA or CSA had more positive symptoms atadmission. The patients reporting CSA had higher scores at Concept of disorder(3.64±1.150:3.01±1.390),hostile(4.04±1.837:2.80±1.443),anxiety(1.72±0.980:2.25±1.326) and impulse control disorders (3.480±1.686:2.65±1.555). The patients withhistory of CEA had more concept of disorder (4.00±1.254:3.02±1.327), exciting(3.73±1.534:2.69±1.345), hostile(4.13±1.457:2.93±1.602), anxiety (1.53±0.743:2.22±1.313),nervous(1.60±0.737:2.38±1.397),attention disorders(3.13±0.743:2.15±1.180)and account for first concept hallucinations (3.93±1.534:2.66±1.816) at admission.4,The patients with history of CEA had more early hospitalizations and moredifficulty in treatment. The patients reporting CSA had more difficulty in learning.60%(22pearsons) the patients with histories of childhood emotional neglect(CEN)reported suicidal attempts.Conclusion:1, We found high rates of childhood trauma among patients with schizophrenia.More attention should be paid to the childhood truma of schizophrenia patients infuture time.2, Childhood trauma may less affect on the schizophrenia, we need furtherresearch to investigate whether child abuse is an independent risk factor forschizophrenia..3, Different forms of childhood truma has different impaction on the patients with schizophrenia, they may need a separate specific analysis to discuss therelationship between schizophrenia and the childhood trauma.
