Objective: To observe the expression of retinoic acid reverse effect onprecancerous lesions of gastric cancer and precancerous lesions of retinoic acidreceptor and retinoic acid intervention significance, thus to discuss the new methodand possible mechanism, in order to provide the theoretical basis for prevention andearly diagnosis of gastric cancer. Methods: Before and after the treatment were selected gastroscopic biopsy.80gastric cancer patients eligible patients, According to visiting time, randomized,Doubleday into treatment group, the control group daily. The ultimate collection: Thetreatment group of50cases, take gastritis therapy (omeprazole and sucralfate) andretinoic acid treatment; the control group of30cases, only take gastritis therapy(omeprazole and sucralfate). Before treatment for pathological evaluationandimmunohisto chemical expression level detection of retinoic acid receptors in thegroup, Withinone week after treatment were repeated once, and compared before andafter treatment the pathological effects and the changes of retinoic acid receptor.Results:1.Medication for three months, Precancerous lesions of gastric cancer wereimproved significantly in the treatment group by the pathological efficacy rate64.00%.Compared with the rate43.33%of the control group, The comparison between twogroups (χ2=6.218, P<0.05). As can be seen that Retinoic acid for the treatment ofprecancerous lesions of gastric cancer effectively.2.The positive rate of retinoic acid receptor rised from66.00%to88.00%. afterthree months treatment. The difference was statistically significant (χ2=6.832, P<0.05),Comparing with the strength, the difference has statistical significance(Z=-2.646,P<0.05). It Showed that retinoic acid can increase the expression of retinoic acid receptors in the tissue, and can enhance the intensity of expression. The control groupbefore and after treatment of retinoic acid positive expression rates were63.33%、66.67%. Is No significant differences(χ2=0.073, P>0.05). Compared with strength, nosignificant difference(Z=-0.693, P>0.05). But the treatment group compared with thecontrol group, retinoic acid receptor positive expression rate and intensity aftertreatment, differences were significant(χ2=5.333, Z=-2.021, P<0.05). It Showed thatretinoic acid can increase the retinoic acid receptor.3.In the pathological response group, the expression of retinoic acid receptorbefore and after the intervention compared, with statistical significance (χ2=11.508,P=0.001, P<0.05); In the no pathological effect group, the expression of retinoic acidreceptor before and after the intervention compared, no statistical significance(χ2=0.000, P=1.000, P>0.05). But the intervention of retinoic acid receptor expressionbetween group comparison, there was statistical Significance (χ2=8.257, P=0.004,P<0.05). These results suggest that, after retinoic acid treatment, the pathologicalaspects have significantly improved, the expression of retinoic acid receptor rateincreased significantly; no significant improvement, the expression of retinoic acidreceptor rate had no significant change, There were positive correlation between. Thatretinoic acid receptor upregulation might be one of the mechanisms of retinoic acidreverse precancerous lesions of gastric cancer.4.In general, three months of treatment with retinoic acid, precancerous lesions ofgastric cancer tissue pathology has been improved, and the number of tissueexpression of retinoic acid receptors, strength increase. With the exception of a fewpatients have stomach discomfort, lips dry hair and other symptoms, and the timelysymptomatic treatment and all were cured, no adverse reactions were observed in theother patients. Conclusion: Retinoic acid can effectively improve the precancerous lesions ofgastric cancer, and can improve the expression of retinoic acid receptor of gastricmucosa. These results suggest that the upregulation of the expression of retinoic acidreceptor may be one of the mechanisms of retinoic acid in the treatment ofprecancerous lesions of gastric cancer. retinoic acid was could effectively reduce theprecancerous lesions of gastric cancer risk, so it can be used for early prevention andintervention treatment of gastric cancer. The whole course of treatment, the liver andkidney, blood routine indexes were normal. It Showed that retinoic acid for thetreatment of precancerous lesions of gastric cancer is safe.
