Objective: Research to the treatment of Retinoic acid on gastric precancerous lesions and its effects on the expression of Rb gene and C-erbB-2 gene, thus to discuss the therapeutic value and possible mechanism, in order to provide the theoretical basis for early diagnosis and treatment of gastric cancer.Methods: By using gastroscopy obtain confirmed by pathology for dysplasia (DYS) and intestinal metaplasia(IM) specimens were 80 cases. 50 cases were randomly divided into the treatment group and 30 cases were divided into the control group. The treatment group were assigned retinoic acid, omeprazole, sucralfate,and the control group were assigned omeprazole and sucralfate. Three months later, the two groups were re-test under gastroscopy, and pathology report were obtained. Rb,C-erbB-2 protein were detected by immunohistochemical S-P method before and after treatment.Results: After the treatment of retinoic acid, gastric epithelial dysplasia and intestinal metaplasia had been improved obviously. The total effective rate in treatment group was 62.00%,and in control group was 36.67%.The effect of pathological change was better in treatment group than in control group. And there was significant difference between them (P<0.05).It showed that gastric precancerous lesions could be cured by retinoic acid. The positive rate of Rb gene(96.00%) after treatment was higher than the positive rate of Rb gene(76.00%) before treatment. And the positive rate of C-erbB-2 gene (8.00%) after treatment was lower than the positive rate of C-erbB-2 gene(34.00%) before treatment. There was significant difference between before treatment and after treatment. And after treatment, the treatment group compared with the control group also had significant difference. After treatment, the expression intensity of Rb gene was better in treatment group than control group. And the expression intensity of C-erbB-2 gene was worser in treatment group than control group. There was significant difference between them (P<0.05).It concluded that retinoic acid should be more effective to the expression of Rb gene and C-erbB-2 gene. After the treatment of retinoic acid, the overall trend showed that with the improvement of gastric precancerous lesions, the expression intensity of Rb Gene was enhanced and C-erbB-2 gene was declined. Few patients appeared headache, dry mouth and abdominal discomfort. After symptomatic treatment, the symptoms were alleviated.Conclusion: It is indicated that retinoic acid had positive effects on gastric precancerous lesions. It could reverse the progression, up regulate the expression of Rb and down regulate the expression of C-erbB-2 in gastric precancerous lesion. This suggested that it was one of the mechanisms of retinoic acid in gastric precancerous lesions. The treatment of retinoic acid to gastric precancerous lesion was safe. A few patients appeared headache, dry mouth and abdominal discomfort. After symptomatic treatment, the symptoms were alleviated. Through follow-up study, liver function, renal function, blood routine test and others indicators were normal. Those showed that retinoic acid was safe and effective.
