A Cross-sectional Study Of The Relationship Between Helicobacter Pylori Infection And Retinoic Acid Metabolism In Residents Aged 40-69 Years In A County With High Gastric Cancer Risk

Objective: The aim of this study was to investigate the status of Helicobacter pylori(H.Pylori)infection and its relationship with retinoic acid(RA)metabolism among residents of 40-69 years old in a county with high gastric cancer risk in Anhui Province.Then this study may provide a population basis for carrying out local effective prevention of gastric cancer(GC).Methods: From July 2015 to July 2018,a cross-sectional survey was carried out in Lujiang County by using stratified random sampling method.We selected 10 administrative villages and communities in Lujiang County,randomly surveyed the local residents aged 40 to 69 years about their general information,lifestyle and behavioral pattern,and dietary intake frequency,as well as physical measurements and venous blood collection.Anti-H.Pylori Ig G was tested by serology.The level of serum retinol was detected by HPLC.RA was detected by ELISA.The gene expression of RA transporter in whole blood was detected by q RT-PCR.We followed H.Pylori seropositive residents who were admitted to the hospital.We collected gastric biopsy tissues from patients with non-atrophic gastritis(NAG),precancerous gastric lesions(PGL)or GC.We performed q RT-PCR,WB and IHC methods to detect the expression of RA synthase(RDH10,ALDH1A1),RA clastic enzymes(CYP26A1,CYP26B1),and one of RA downstream signaling pathway receptor,RAR?,in NAG,PGL and GC.Results: A total of 1392 local residents aged 40-69 years in Lujiang County were included in this study,430 people were male(30.89%)and 962 were female(69.11%).The positive rate of H.Pylori was 50.07%.There were significant differences in age,BMI,education,work,income,and H.Pylori infection between different genders(P<0.05).The results of H.Pylori infection influence factor analysis showed that BMI of 24-27.9(P<0.001,OR=1.689,95%CI 1.270-2.245),high salt intake(P=0.021,OR=1.462,95%CI 1.060-2.015)and high frequency intake of salted food(P=0.040,OR=1.560,95%CI 1.021-2.383)were influence factors for H.Pylori infection in female;drinking(P=0.003,OR=1.834,95%CI 1.228-2.739)was the influence factor for H.Pylori infection in male.There was no statistical difference in the intake frequency of vitamin A-rich foods under different H.Pylori infection(P>0.05).The average serum retinol concentration of H.Pylori-positive participants was 0.936?0.419 ?g/m L,and that of H.Pylori-negative participants was 1.103?0.509 ?g/m L.However,there was no significant difference between the two groups(P=0.139).The average serum RA concentration of H.Pylori-positive subjects was 4.648?2.779 ng/m L,and that of H.Pylori-negative subjects was 4.320?2.838 ng/m L.There was no significant difference between the two groups(P=0.368).The m RNA expression of RA transporter,cellular retinoic acid binding protein 1(CRABP1),in serum in H.Pylori-positive individuals was higher than which in H.Pylori-negative participants(P<0.05),while the m RNA expression of CRABP2 was decreased(P<0.05).Among the tracked H.Pylori-positive individuals,compared with NAG and PGL,the m RNA and protein expression of RA synthetase RDH10 and ALDH1A1 were significantly reduced in GC(P<0.05),while the expression of CYP26B1 was significantly increased in GC(P<0.05).The m RNA and protein expression of RAR? in GC tissue was decreased compared with NAG and PGL(P<0.05).But,the protein expression of RAR? in PGL tissue was significantly reduced compared with NAG(P<0.05).Conclusion: Residents from this county with high gastric cancer risk may reduce H.Pylori infection by preventing overweight,high salt intake,high frequency intake of salted food and drinking.Although this study had not been found that the levels of RA and its precursors are related to H.Pylori infection,the transport and metabolism of RA were already abnormal in H.Pylori-positive individuals,which may affect the downstream signaling pathway of RA and cause its impaired function.
