Digestive Endoscopy And Laparoscopy In The Treatment Of Gastric Stromal Tumors And To Compare The Clinical Value

Objective To evaluate the clinical application of feasibility of digestive endoscopic treatment for gastric stromal tumors,by comparing the efficiency and security of Endoscopic and laparoscopic in resecting of gastric stromal tumors.Methods A retrospective and comprehensive analysis is made based on the clinical material of endoscopic (53cases)and laparoscopic (39cases) resection for gastric stromal tumors(diameter<3.0cm with clear boundary),by comparing the operation time,intraoperative blood loss, postoperative gastric tube indwelling time,recovery of bowel function time, postoperative complications, postoperative hospital stays, recurrence rate,metastasis.Results Compared with the laparoscopic group, the endoscopic group present shorter operation time [50(48～58) min比70(50～95) min, U=1575.00, P<0.01], less intraoperative blood loss [10(5～15) ml比20(20～30) ml, U=1794.00, P<0.01], early recovery of bowel function time [18(8～36) h比24(20～40) h, U=1666.00,.P<0.01]. The postoperative gastric tube indwelling time, postoperative complications, postoperative hospital stays had no statistical difference (P>0.05). Compared postoperative follow-up time with the laparoscopic and endoscopic groups were(27±15) and (24±11)months (t=0.3084, P>0.05). In the endoscopic group, it has no tumor recurrence or distant metastasis, nor did a death for gastric stromal tumors during the follow-up time.Conclusion The excision operation of endoscopy upon the gastric stromal tumors whose diameter is less than3.0cm with clear boundary is safe, efficiency, security,compared with laparoscopic surgery.It has little pain and quickly recovery.
