Objective By comparing lung cancer brain metastases and high-grade gliomas parenchyma, necrosis, peritumoral edema apparent diffusion coefficient (ADC) value, to explore the value of DWI in brain metastases of lung cancer and the identification of high-grade gliomas.Methods Collection were29cases of lung cancer brain metastases and12cases of high-grade gliomas (WHO grade gliomas:Ⅲ-Ⅳ grade) head MR data (including conventional unenhanced, enhanced scan and DWI imaging), a retrospective analysis of lesions Imaging performance. In the tumor parenchyma, necrosis, peritumoral edema (swelling outside near the tumor area with enhanced tumor less than10mm, much enhanced tumor edema tumors outside the region within10-20mm) were selected three-five regions of interest, taking the average, that ADC values and statistical analysis.Results Multiple brain metastases of lung cancer in20cases,9cases of solitary. Figure10cases of lesions on DWI were seen high signal, the signal on the ADC map the opposite case, showed low signal. Visible low signal on another19cases DWI lesion center map, ADC photo shows high signal. High-grade gliomas are single. Figure five cases of lesions on DWI see high signal, and the other seven cases no obvious high signal on ADC maps hypointense lesions in five cases, and the other seven cases seen high signal. Lung cancer brain metastases and high-grade gliomas parenchyma ADC values were0.86±0.23,1.07±0.14, the difference was not statistically significant (t=-1.863, p value0.085); Necrosis ADC values were2.41±0.47,2.83±0.08, the difference was statistically significant (t=-3.696, p=0.001); Near the tumor edema ADC values were1.58±0.18,1.41±0.16, the difference was statistically significant (t=2.877, p=0.006); distant tumor edema ADC values were1.59±0.08,1.51±0.15, the difference was not statistically significance (t=1.787, p value0.096). Near tumor metastases edema, edema distant tumor ADC values were 1.58±0.18,1.59±0.08, the difference was not statistically significant (t=-0.322, p value0.742); high-grade glioma tumor edema near, far tumor edema ADC values were1.41±0.16,1.51±0.15, the difference was statistically significant (t=-20.742, p=0.000).Conclusion1. lung cancer brain metastases and high-grade gliomas parenchyma ADC values can not provide the basis for the differential diagnosis.2. ADC values necrosis of lung cancer brain metastases and high-grade gliomas of the latter than the former, can provide the basis for the differential diagnosis.3. lung cancer brain metastases and high-grade gliomas near peritumoral edema ADC values can provide more information for identification.4. ADC value of high-grade gliomas peritumoral edema was lower than brain metastases, and have different distances, the greater the distance the higher the ADC value.5. In conventional MRI plain and enhanced performance basis, through necrosis, edema measurement and analysis of ADC values conducive to brain metastasis of lung cancer diagnosis and differential diagnosis of tumors and high-grade gliomas, and in defining tumor border areas to provide more useful information.
