Objectives Through researching expression of CD40/CD40ligand(CD40L) system and CD134/CD134ligand(CD134L) system on CD4+T lymphocytes in patients with CHD to investigate functions of immunoglobulins of CHD and using different concentrations of cyclosporine A (CSA) to intervene expression of CD134/CD134L and CD40/CD40L system to provide effective data for the treatment of CD40/CD40L system and CD134/CD134L system as a target, as well as develop new ideas and methods for the therapy of CHD with immune inhibitors.Methods71patients with CHD from the Department of Cardiology of Tianjin Chest Hospital were divided into4severity different groups according to clinical manifestations and ECG:acute myocardial infarction (AMI) group(19case),unstable angina pectoris(UAP) group(18case),17cases of stable angina pectoris (SAP) group and17normal patients as control group (N).Testing blood biochemical indexes and collecting the basic data from all the patients,using density-gradient centrifugation to isolate Peripheral blood mononuclear cells(PBMCs),using flow cytometry to analysis the expression percentage of CD40/CD40L and CD134/CD134L system on CD4+T lymphocyte in each group and screening the group of the highest expression percentage,by adding different concentrations of CSA (concentration gradient:0,0.1,0.01,10μg/mL) stimulate the expression on the highest expression percentage group, after72hours analysis the expression percentage by flow cytometry again.At the single cell level evaluate the effection of CSA on expression of the CD40/CD40L and CD134/CD134L system on CD4+T lymphocyte.Data were analyzed through SPSS software(version18.0), P<0.05is of statistically significant difference.Results (1)some indexes of age, LDL-C,Gensini coronary scoring system, FT3and HDL-c have statistically significant difference (P<0.05), including age, LDL-C, Gensini coronary scoring system increased sequentially in different severity groups, FT3and HDL-c in different severity groups are descending. There were no significant differences between the groups of other basic data (P>0.05).(2)The expression of CD4:N group compared with SAP group, UAP group and AMI group, CD4expression were increased with the increase of lesion degree, the difference was statistically different(P=0.001); The expression of CD40showed that SAP group, UA group, AMI group and N group were statistically significant different(P=0.001), there were significant difference among SAP group and UA group、AMI group (P<0.05), but there were no significant differences among UA group and AMI group (P>0.05); the expression of CD40L showed that SAP group, UA group and the AMI group increase significantly than N group, the expression percentage increase in turn in different severity group, differences were significant between both group(P=0.001); the expression of CD134show that AMI group and UA group were higher than N group and have statistical significance difference(P<0.05), therer are no significant difference among the rest groups(P>0.05); the expression of CD134L showed that therer are no significant difference among both groups(P=0.17).(3)Correlation analysis:Positive correlation were found among CD40,CD40L and Gensini coronary scoring system(P=0.001),with the Gensini integral increases CD40,CD40L expression increased.While there were no correlations between CD134,CD134L and Gensini coronary scoring system(P=0.693/0.892).(4)the expression of CD40:CSA intervention concentration0.1μg/mL and1μg/mL group were significantly lower than0μg/mL,0.01μg/mL group (P<0.01), therer are no significant difference among the rest groups(P=0.076/0.552);there were statistically difference about expression of CD40L in CSA intervention concentration Ong/mL,0.01μg/mL and0.1μg/mL groups (P<0.05), the expression was gradually decreased with the increase of the concentration of CSA, but0.1μg/mL and1μg/mL group showed no significant difference(P=0.585); CD134and CD134L expression in different group were decreased as the increasement of CSA intervention concentration,but therer are no significant difference among both groups(P=0.779/0.721).Conclision CD40,CD40L, CD134, CD134L can be founded in CD4+T lymphocytes of nonnal person and patients with CHD,their expression maybe participate in the formation and development of CHD through mediating inflammatory immune response,but CD134,CD134L show no effection.CSA inhibit the formation of atherosclerosis through inhibit the expression of CD40/CD40L system of AMI CD4+T lymphocyte, but had no effection on CD134/CD134L system, the mechanism is still not clear, so can not determine the therapeutic effect of CSA on CHD.
