Objective:By comparing duloxetine combination with alpha-lipoic acid、duloxetine and alpha-lipoic acid in the therapeutic effect of painful diabetic neuropathy, to explore the efficacy and safety of duloxetine combination with alpha-lipoic acid treatment on diabetic painful neuropathy is optimal.Methods:90cases hospitalized in our department with painful diabetic neuropathy of type2diabetes were randomly divided into three groups by digital cards, which were duloxetine combination with alpha-lipoic acid group, duloxetine treatment group,and alpha-lipoic acid treatment group. Each group has30patients. Before treatment and1,4,8weeks after treatment, median nerve sensory conduction velocity, sural nerve sensory conduction velocity, the NRS and VRS score and rating are compared.Results:1.Before treatment,gender,age,course.FPG,HbAlc,low-density lipoprotein (LDL). triglyeride (TG). systolic pressure. diastolic blood pressure. median nerve sensory conduction velocity, sural nerve sensory conduction velocity, the NRS and VRS score and rating differences had no statistical significance in three groups (P>0.05).2.The median nerve sensory conduction velocity and sural nerve sensory conduction velocity of therapeutic alliance team is better than its before treatment(p <0.05) after one week,four weeks and eight weeks.The improved nerve conduction velocity of duloxetine combination with ALA group after four weeks is better than its after eight weeks(p<0.05).The nerve conduction velocity of duloxetine combination with ALA group after eight weeks is close tonormal.3.The median nerve sensory conduction velocity and sural nerve sensory conduction velocity of duloxetine treatment group has no significant difference after one week and four weeks (p>0.05).4.The VRS and NRS score of duloxetine treatment group after four weeks is significantly lower than its after one week,which has significant difference(p<0.05), and the score after eight weeks is better than its after four weeks(p<0.05).The median nerve sensory conduction velocity and sural nerve sensory conduction velocity of therapeutic alliance group has no significant difference among one week,four weeks and eight weeks (p>0.05).Compared with duloxetine treatment group after one week, four weeks and eight weeks of treatment.therapeutic alliance team has a good conduction of velocity on median nerve and sural nerve. After4and8weeks,the VRS and NRS score has significant difference between therapeutic alliance group and duloxetine treatment group (p>0.05),and the score of herapeutic alliance group is lower than duloxetine treatment group.After one and four weeks.the VRS and NRS score of therapeutic alliance group is more lower than ALA treatment group(p<0.05).Conclusion:1.Duloxetine has a good analgesic effect and security.but has no effect of neuroprotection and reversing pathogenesis on PDN.2.The study demonstrates ALA can block the pathways of oxidative stress and reduce the level of radical oxidative stress in vivo.ALA has significant effect of improvement on nerve conduction velocity in patients with PDN.The PDN patients of ALA treatment in four weeks received clinically significant improvement in symptoms.Eight weeks for continued treatment is regarded as a course of treatment with PDN.3.Duloxetine combined ALA has a good analgesic effect and good security on treatment PDN.It can be used as an ideal PDN treatment protocols.
