Objective:To establish an evaluation system that was new, accurate, scientific, simple and convenient. This system can not only properly reflect the clinical significance of detection of hepatitis B virus markers, but also help inspectors and clinicians to diagnose the disease simply, and to analysis the prognosis of the disease. Thereby, the "two half and half assay second liver can be well applied in clinical.Methods:1.The patients who have been checked to HBsAg, HBsAb, HBeAg, HBeAb and HBcAb (two half and half) in Dalian Central Hospital and the Second People’s Hospital of Dalian were selected according to the following criteria:①AST/ALT<40U/L, HBsAg-negative, and at least one of those HBsAb, HBeAb, HBcAb was positive.②AST/ALT>40U/L, HBsAg-positive, and at least one of those HBsAb, HBeAb, HBcAb was positive.③To remove the patient who has the HBsAb-positive only.This study included213patients aged (65±12.7) years according to the criteria, and there were107males and106females. These patients were divided into normal liver enzymes group (n=107,55males,52females) and abnormal liver enzymes group (n=106,54males,52females) according to the AST and ALT.There are507healthy people who have been checked the "two half and half assay second liver in Dalian Central Hospital and the Second People’s Hospital of Dalian were selected randomly as control group.2.That the ROC analysis, scatter plot, rank sum test and other statistical methods being used to establish the cutoff value of HBsAb, HBeAb, HBcAb are as follows:①The HBsAb<9.9IU/L, the value of antibody index was1; The HBsAb>9.9IU/L, the value of antibody index was0.②The HBcAb<0.01COI, the value of antibody index was0; The HBcAb>0.01CO I, the value of antibody index was1.③The HBeAb<1.8COI, the value of antibody index was0; The HBeAb>1.8COI, the value of antibody index was1.④The antibody index can be defined as:the value of the antibody index equaled the summation of the values of HBsAb, HBcAb, and HBsAb. And calculate the value of antibody index, to establish the evaluation system of antibody index.3.The antibody index evaluation system was used to summarize the results based on clinical application.Result:1. These three kinds of cutoff values of antibody and antibody index were:①The HBsAb<9.9IU/L, the value of antibody index was1; The HBsAb>9.9IU/L, the value of antibody index was0.②The HBcAb<0.01COI, the value of antibody index was0; The HBcAb>0.01COI, the value of antibody index was1.③The HBeAb<1.8COI, the value of antibody index was0; The HBeAb>1.8COI, the value of antibody index was1.④The antibody index can be defined as:the value of the antibody index equaled the summation of the values of HBsAb, HBcAb, and HBsAb.2.If the liver enzymes were normal, there are57%possibilities suggested a well recovery or a best period of immunity when the antibody index was0;36.4%possibilities suggested a trend of well recovery or a moderate period of immunity when the antibody index was1;6.5%possibilities suggested prognosis mala when the antibody index was2; and it suggested no antibody production when the antibody index was3. If the liver enzymes were abnormal, there are1.9%possibilities when the antibody index was0;67.0%possibilities suggested a trend of well recovery when the antibody index was1;28.3%possibilities suggested a weak immunity and prognosis mala when the antibody index was2; and2.8%possibilities when the antibody index was3.Conclusion:Hepatitis B immune reaction which was indicated by the antibody index can accurately illustrate the relationship between hepatitis B infection and immune response, and it also can judge the prognosis.
