The Correlation Research Of Recurrent Of Calcium Oxalate Urinary Stones And Damp Heat Constitution

ObjectiveThe recurrence rate of urolithiasis is very high in clinical, the treatment of urolithiasis has made rapid development at present, but we still lack of correlation research of preventive of urolithiasis recurrence. The study based on the correlation research of the causes of urolithiasis and TCM consititution, to reveal whether there is a link between postoperative with calcium oxalate urinary stones and damp heat constitution in retrospective analysis, so that we could provide some certain clinical evidence for better pervention of urolithiasis recurrence.MethodsWe selected1550inpatients with urolithiasis of guangdong provincial hospital of TCM during January2010to December2013, a total of84postoperative patients with recurrence of calcium oxalate urinary stones were included in the standard. All the patients taked the TCM constitution classification and identified them into group of damp heat constitution and group of non-damp heat constitution. Collected the patient’s general information, the position of stone recurrence, the period of stone recurrence,renal function,urinary tract infection, etc., then using statistical analyze methods and made the conclusion.Results1. In84postoperative patients diagnosed as recurrence of calcium oxalate urinary stones, including52men(62%), whose ages between28-80, the average age is (53.04±11.83). And32women(38%), whose ages between35-84, tha average age is (53.72±10.73)2. Analyze of the period of stone recurrence:there were22cases of recurrence after1year,51cases of recurrence after3years,73cases of recurrence after5years,82cases of recurrence after8years.5of them recurrence twice. The period of recurrence between2-108months. The average time of recurrence is33.7months. And the average period of postoperative recurrence of ureteral calculi is29.7months, the average period of postoperative recurrence of kidney stone is44.5months, the average period of postoperative recurrence of bladder stone is29.7months.3. Analyze of the rate of stone recurrence:The study collect1550patients with urinary stones, including677patients with ureteral calculi,655patients with kidney stone, and218patients with bladder stone. And84postoperative patients with recurrence of calcium oxalate urinary stones are included in the standard, including58postoperative recurrence patients with ureteral calculi,19postoperative recurrence patients with kidney stone, and7postoperative recurrence patients with bladder stone. We can found that the average rate of postoperative recurrence of ureteral calculi is8.6%,the average rate of postoperative recurrence of kidney stone is2.9%, the average rate of postoperative recurrence of bladder stone is3.2%.4. All84postoperative patients with recurrence of calcium oxalate urinary stones taked the TCM constitution classification,, there are49cases of damp heat constitution and35cases of non-damp heat constitution. So damp heat constitution is the high risk of recurrence of urinary stones.5. Analyze of the gender composition of84stone recurrence patients:men of damp heat constitution is more than non-damp heat constitution, and women of damp heat constitution is almost the same with non-damp heat constitution. From the overall, man is more likely to relapse urinary stones when compares with woman.6. Analyze of the age composition of84stone recurrence patients: damp heat constitution most appeared in the middle-aged and young patients, and non-damp heat constitution most appeared in the middle-aged and old patients. Two groups of patients compares chi-square test has a significant difference(P<0.05) in the structure of age, which shows that the age has certain correlation with urinary stones recurrence.7. Statistic the number of urinary stone recurrence patientsd in ifferent group of TCM constitution, we found that the number of ureteral calculi recurrence are most of all, the second of all are the number of kidney stone recurrence, and the number of bladder stone recurrence are the least. The rate of recurrence of ureteral calculi in both side are equal,so as the kidney stone. Because the number of bidder stone recurrence is little, so we not included in the statistics.8. There are no obvious difference in the indicators of renal function and urinary tract infection of two groups, but most of the patients of ureteral calculi recurrence have higher serum creatinine.9. Statistic the situation of postoperative residue stones, both ureteral calculi and bladder stone have less residue stones, the kidney stone has more residue stones.Three groups of patients compares chi-square test has a significant difference(P<0.05).10. Personal past health history has no effect in the currence of urinary stones,but all the patients of bladder stones recureence have hyperplasia of prostate.Conclusion1. In all the postoperative patients with recurrence of calcium oxalate urinary stones, people in damp heat constitution is more than non-damp heat constitution, the number of man is more than women, and the damp heat constitution most appeared in the middle-aged and young patients, and non-damp heat constitution most appeared in the middle-aged and old patients.2. Statistic the number of urinary stone recurrence patientsd in ifferent group of TCM constitution, the number of ureteral calculi recurrence are most of all, the second of all are the number of kidney stone recurrence, and the number of bladder stone recurrence are the least. Analyze of the period of stone recurrence, the average period of postoperative recurrence of ureteral calculi is shortest, the second is bladder stone, and the period of kidney stone recurrence is the longest. Analyze of the rate of stone recurrence, the average rate of postoperative recurrence of ureteral calculi is highest, the second is bladder stone, and the rate of kidney stone recurrence is lowest.3. Both ureteral calculi and bladder stone have less residue stones, the kidney stone has more residue stones. Postoperative residue stones will increase the probability of recurrence. Most of the patients of postoperative recurrence of ureteral calculi have higher serum creatinine. All the patients of bladder stones recureence have hyperplasia of prostate.
