ObjectiveAfter the fracture happened, due to the body’s stress response the procoagulant factors are released into the blood, and cause the blood hypercoagulabale state, finally resulting in the formation of deep vein thrombosis. Nowadays the traditional of hypercoagulable state treatments have lots of adverse reactions and led to Restrict in its use. so this study was to find a better intervention measure, which has little adverse reaction and can be used in earlier period after the fracture occurred. Through the use of traditional Chinese medicine, the following parameters were measured, including plasma D-dimer、whole blood platelet counts(PLC) and Prothrombin time(PT), to find the differences in medical treatment and traditional treatment methods and the effectiveness of chinese medical treatment. Thus to seek the ideal clinical interventions.MethodsFrom November,2013to February,2014, the patients of femoral shaft fracture in elderly were checked when they just stayed in hospital. Total50patients are divided into two groups which was based on the intervention measure,25cases of each group. The following parameters were measured when the first day of admission, including plasma D-dimer、whole blood platelet counts (PLC) and Prothrombin time (PT). and the next day, group A used the medicine of Buyang Hangwu Tang, Group B used the medicine of low molecular weight heparin, all the patients’treatment from the next day to the seventh days. The plasma D-dimer、whole blood platelet counts(PLC) and Prothrombin time(PT) were measured on the first, the third and the seventh day. Also observed before and after treatment in patients with the tongue, Sublingual venae, pulse. Finally we test results and statistical analysis.Results(1)When two groups of patients were on admission, D-dimer of group A were615.84μg/L±151.22μg/L, group B were627.54μg/L±159.65μg/L, Gradually decreased after treatment, To3days, Dropped to527.48μg/L±108.0μg/L、517.36μg/L±124.75μg/L. To7days, Dropped to353.88μg/L±104.34μg/L340.03μg/L±102.89μg/L. According to the statistical analysis, there was no statistical difference between Group A and Group B.(2) When two groups of patients were on admission, the PLC of group A were224.96X109/L±48.53X109/L, group B were217.72X109/L±32.91X109/L X109/L, Gradually decreased after treatment, To3days, Dropped to243.80×109/L±34.42X109/L、248.12X109/L±29.06X109/L. To7days, Dropped to252.96×109/L±25.91X109/L、250.56X109/L±35.50X109/L. According to the statistical analysis, there was no statistical difference between Group A and Group B.(3) When two groups of patients were on admission, the Prothrombin time(PT) of group A were10.69s±0.68s, group B were10.76s±1.31s,09/L, Gradually increased after treatment, To3days, change to11.43s±0.44s,11.4s5±0.43s. To7days, Dropped to11.41±0.43,11.47±0.51. According to the statistical analysis, there was no statistical difference between Group A and Group B.ConclusionAfter femoral shaft fracture happened, the procoagulant factors are released into the blood and cause the blood hypercoagulabale state. According to TCM theory, fractures often result in qi deficiency and blood stasis. And the Buyang Huanwu Tang can reduce the plasma D-dimer, lower the incidence of DVT and hypercoagulable state, can treat the qi deficiency and blood stasis. So the Clinical observation of50patients found that the treatment between Buyang Huanwu Tang and low molecular weight heparin is almost the same. However there is no obviously adverse reaction of Buyang Huanwu., so it is particularly applicable to the early pos-fracture treatment of hypercoagulable state.
