Objective: To observe the change of glomerular charge barrier in different stageof diabetic nephropathy and analyse related influencing factor and in order topost a direction for clinical diagnosis and treatment of DN.Methods: According to the urinary albumin excretion rate and renal function,one hundred and two hospitalized patients with diabetes mellitus were dividedinto four group:36diabetic patients with normal urinary albumin excretion,32patients with DN in stage III,21patients with DN in stage IV and13patientswith DN in stage V. And the control group of40patients was choosed from thephysical examination center with similar age and sex. Then pancreatic amylaseand salivary amylase of serum and urine were checked and CPAM/CSAMwascalculated. Next, the means of CPAM/CSAMamong different groups were compared, and the relationship between CPAM/CSAMand HbA1c was Analysed.Result:There was a significantly different among three groups of DM withnormal urinary albumin excretio, DN in stage III and DN in stage IV. There wasno significantly different between the group of DM with normal urinary albuminexcretio and control group. And there was no significantly different between thegroups of DN in stage IV and DN in stage V. Moreover, there was a negativecorrelation between the value of CPAM/CSAMand HbA1c.Conclusion: As the development of DN, the damage of glomerular chargebarrier become worse and worse, and in a certain range,reduction of HbA1cmay delay the damage of glomerular charge barrier.
