Comparison Between Compound Lidocaine Cream Applied Over Endotracheal Tube And Ketamine Gargle For Attenuating Postoperative Sore Throat

Abstract/Summary:
ObjectivesThis study compares the incidence of postoperative sore throat after general tracheal anesthesia when applying compound lidocaine cream on the tracheal tube or ketamine gargling, to seek the better way for prevention and treatment of postoperative sore throat.MethodsAltogether186patients undergoing elective surgical procedures under general anaesthesia, woman, aged20~60yr,American Society of Anesthesiologists physical status class Ⅰ or Ⅱ or Ⅲ,, BMI<25, after entering the operating room, they were randomly allocated into three groups according to draw lots. Compound Lidocaine cream group(group L, n=62),Ketamine gargle group (group K, n=62),Control group(group C, n=62).Orotracheal intubation was performd in all the Patients.5minutes before induction, compound lidocaine cream was uniformly smeared on the formost third of endotracheal tubes before intubation in group L; ketamine50mg in saline30ml, patients need to gargle this mixture for60seconds in group K, no drug applied in group C. The changes in blood pressure and heart rates during intubation, the Ramsay scores in PACU(post anesthesia care unit), bucking、breath holding and sore throat during extubation,wound pain VAS(visual analog scale,VAS)and the POST grades at0h,1h,6h,12h and24h in the postoperative period after tracheal extubation were assessed.ResultsThe MAP and HR were significantly lower after induction of anesthesia compared with baseline blood pressure,but there was no significantly difference among the three groups.During intubation, the fluctuations of MAP and HR were not obvious. There was no significantly difference on Ramsay scores among the three groups in PACU. During tracheal extubation, compared with group C, the incidence of bucking and breathing holding were lower in group K and L,but there was no significantly difference between the group K and group L. After extubation,the highest incidence of POST occurred at6hours in all groups, In the first24h after extubation, the incidence of POST was50.8、31.2、50.2%for the group L、Kand C. The incidence of POST was significantly lower in the Ketamine group compared with the other two groups (P<0.05), there was no significantly difference between group C and group L for the incidence and severity of POST(P>0.05). Between groups comparisons of VAS score showed no significant difference at any time point.Conclusionscompared with the group L and K, Ketamine group had significantly decreased the incidence and severity of POST,compound lidocaine cream may have no effect on reducing the incidence and severity of POST.There was no obvious side effect of drugs.
