Objective:To investigate the expression of PLTP and the changes of blood lipid in Intrahepatic cholestasis Of pregnancy patients and their relationship with pathogenesis of Intrahepatic cholestasis Of pregnancy.Method:30patients with Intrahepatic cholestasis Of pregnancy were recruited as the research group (ICP group),20patients were mild,10patients were severe; and20healthy pregnant women were defined as the control group (Normal group). Enzymatic circulating method was used to investigate the levels of total bile acid, oxidase-method was used to investigate the levels of HDL and LDL, Homogeneous enzymatic was used to investigate the levels of CHOL and TG, Enzyme-linked immuno sorbent assay (ELISA) was used to investigate the levels of PLTP, Real-time semi-quantitative reverse transcription (RT-PCR) was used to investigate the expression of PLTP in the placental tissue. The expression of PLTP and blood lipid changes were compared between the research group and the normal group, and their relationship were studied.Result:1. The levels of lipid in the ICP group:the levels of TBA、CHOL、 LDL> TG in the ICP group were (32.49±20.68) ummol/L、(4.67±1.88) mmol/L.(3.64±1.27) mmol/L、(2.68±0.89)mmol/L, significantly higher than that of the normal group (3.68±2.21) ummol/L、(2.81±0.78) mmol/L.(2.40±0.60) mmol/L、(1.82±0.32) mmol/L, while the level of HDL in ICP group was (1.24±0.33) mmol/L, significantly shorter than that of the normal group (1.94±0.41) mmol/L, the differences were statistically significant (P<0.001). The levels of TBA、CHOL、LDL、 TG in the severe ICP group were (52.31±13.06) ummol/L、(6.10±1.27) mmol/L、(4.45±1.05) mmol/L、(3.30±0.76) mmol/L, significantly higher than that of the mild ICP group (16.35±6.87) ummol/L.(3.15±1.02) mmol/、(2.71±0.90) mmol/L、(2.01±0.48) mmol/L; while the level of HDL in the severe ICP group was(1.16±0.30) mmol/L, significantly shorter than that of the mild ICP group(1.31±0.32)mmol/L, and they were associated with the severity of the disease. The differences were statistically significant (P<0.001)2. The level of PLTP in the plasma of the ICP patients was (5.13±1.39) pg/ml, significantly higher than that of the normal group (2.74±0.18) pg/ml, The level of PLTP in the plasma of the severe ICP patients was (6.28±1.07) pg/ml, significantly higher than that of the mild ICP group (3.93±0.29) pg/ml, and it was associated with the severity of the disease. The differences were statistically significant (P<0.001)3. The expression of PLTP in the placental tissue of the ICP patients was (15.13±3.39), significantly higher than that of the normal group (2.89±1.05)。The expression of PLTP in the placental tissue of the severe ICP patients was(16.32±4.15), significantly higher than that of the mild ICP group(7.47±2.50), and it was associated with the severity of the disease. The differences were statistically significant (P<0.001)4. In the ICP group, CHOL. LDL、TG and TBA were significantly positive correlated (r=0.860, P<0.01; r=0.724, P<0.01; r=0.782, P<0.01;), HDL and TBA were significantly negative correlated (r=-0.615, P<0.01).5. In the ICP group, The expression of PLTP in the placental tissue and plasma were significantly positive correlated, r=0.897, P<0.01。 The level of PLTP in the plasma were significantly positive correlated with TBA、CHOL、LDL、TG(r=0.959, P<0.01; r=0.815, P<0.01; r=0.692, P<0.01; r=0.758, P<0.01), and it was significantly negative correlated with HDL (r=-0.598, P<0.01).6. In the ICP group, CHOL、HDL、PLTP were the factors that affect the level of TBA, the regression equation was Y=-33.7+2.329XCHOL-1-876XHDL+10.895XPLTp, R2=0.961,(P<0.01)Conclusion:1. In the ICP patients, the levels of CHOL、LDL、TG were significantly higher, while the level of HDL was lower, and they were associated with the severity of the disease, it prompted that disorder of lipid metabolism existed in the ICP patients. 2. PLTP was highly expressed in the placental tissue and plasma of the ICP patients, which affected the metabolism of the lipid、bile acid and the pathogenesis of Intrahepatic cholestasis Of pregnancy.
