| Purpose and significance:This study is based on the theory "exterior and interior relation of Lung and Large Intestine". By observing the clinical efficacy, adverse reactions and improvement of the quality of the patients’life of the study that tongfu Xiezhuo and brucea javanica oil emulsion are both injected to chest pleural effusion of lung cancer patients, thus to explore new ideas and methods of treatment of lung cancer patients who have pleural effusion, and provide the basis for the treatment of lung cancer by tongfu Xiezhuo pleural effusion, and ultimately to improve the quality of life of patients with pleural effusion of lung cancer and prolong survival purposes.Research Methods:Clinical studies was used. Methods established inclusion criteria, exclusion criteria, the criteria of termination of the experiment standards.40cases of lung cancer patients were collected, they were randomly divided into treatment group and control group. Both groups underwent closed chest tube drainage, drainage was generally not more than800ml at the first time, continuous drainage was generally3-5days, not more than1000ml, confirmed by the B-pleural effusion, start line drug treatment. Treatment group and the control group were injected with saline slowly javanica oil10ml plus60ml, in order to reduce the biological effects of stimulus-response modifier pleura, patients were filling2%lidocaine5mg, then clipping catheter. After each injection and supine patient was asked to change position, so that the drug had extensive contacts with the pleura, every other day, for5consecutive times for a course of treatment. The treatment group tongfu Xiezhuo prescription orally, twice a day, and observed the adverse reactions of the treatment. Before and after treatment, water chest was determined by B-line, and liver and kidney function observed changes in blood, carcinoembryonic antigen, DNA aneuploidy and improvement of clinical symptoms to evaluate the therapeutic effect, and followed up for4to6weeks.Results:In terms of total efficiency, total effective rate was76.2%; control group was68.4%, the two groups were statistically no significant difference test (P>0.05), stable after treatment, the treatment group19.0%10.5%in the control group, the two groups showed no significant difference in stability (P>0.05) by the statistical test. In terms of the efficacy of the treatment group had slightly better than the control group. After treatment in the control group KPS score improvement rate of81.0%, the improvement rate of68.4%in the control group. Comparison of the two groups showed no significant difference in improvement (P>0.05) by the statistical test.Treatment group healed in6cases,14cases markedly effective one case, Invalid0patients in the control group, the clinical cure three cases, nine cases markedly effective in6cases, one case, the efficacy of TCM syndrome groups were significantly different statistical test (P<0.05). Especially in improving chest tightness, shortness of breath, coughing symptoms, the treatment group than the control group. In the adverse reaction, the treatment group had four cases of chest pain, fever in1case. Control group had five cases of chest pain, two cases of fever, the two groups were no serious adverse reactions, fever caused by the two groups, no significant difference in chest pain (P>0.05). And in the pleural fluid CEA, DNA aneuploidy, survival aspects, no significant difference in the statistical test (P>0.05), but the treatment group had better trend in the control group.Conclusions:The treatment of lung cancer and malignant pleural effusion perfusion by Tongfu Xiezhuo basic side joint javanica oil emulsion, and has little adverse reaction, can improve clinical symptoms and improve the quality of the patients’lift with advanced lung cancer.So it can be as a pleural effusion kinds of treatment,.And it is worth to apply clinically and study further. |
