Objective:The objective is to evaluate the use of the harmonic scalpel in thyroid surgery.Method:The literatures were collected by searching pubmed、EMBASE、Cochrane Library、web of science,CNKI about the use of the harmonic scalpel in thyroid surgery. The the literatures were included according to the inclusion and exclusion criteria and analyzed by RevMan5.2.0o Results:Finally13randomized controlled trials (n=1517)were included.Meta-analysis showed that:compared with traditional surgical methods,the use of ultrasonic scalpel can reduce the operation time of total thyroidectomy[WMD=-22.73,95%CI(-29.15~16.32,P<0.00001)] and lobectomy[WMD=-16.21,95%CI(-19.74~12.69, P<0.00001)]and decrease the amount of intraoperative bleeding[WMD=-31.00,95%CI(-38.85~-23.15,P<0.00001)], postoperative drainage in total thyroidectomy [WMD=-12.56,95%CI(-21.36~-3.76, P=0.005)]and transient hypocalcemia[OR=O.59,95%CI(0.42~0.83, P=0.003)].But there are no statistically significant differences in recurrent laryngeal nerve palsy.Conclusion:The use of ultrasonic scalpel can reduce the operation time of total thyroidectomy and lobectomy and decrease the amount of intraoperative bleeding, postoperative drainage in total thyroidectomy and transient hypocalcemia. |
