Objective To explore the clinical features of patients with inflammatorybowel disease (IBD) in General Hospital of Ningxia Medical University in recent10years.Methods374cases of hospitalized IBD patients admitted in GeneralHospital of Ningxia Medical University from January2003to December2003were enrolled in this study to collect the related clinical data. The clinicalcharacteristics are analyzed and summarized, such as clinical type, severity,disease staging, area of lesions, complications, endoscopic and pathologicalperformance.Results1. IBD patients of the recent10years is increasing yearly.A total of374hospitalized IBD patients were admitted in recent10years.Only19cases of IBD patients were admitted in2003, and the number had risen to55in2012, increasing by about1.89times. There was an upward trend in IBDhospital admissions.2. Clinical characteristics of UCAmong the374cases,306cases were with ulcerative colitis (UC). The meanaverage age of the UC patients (male-female ration of1.4:1) was46.5years, andthe smokers accounted for24.18%.The main manifestations of UC patients in digestive system weremucopurulent bloody stool (89.2%), diarrhea (45.4%) and abdominal pain (36.3%). The systemic manifestations include fever (12.1%) and weight loss (6.9%).8cases(2.6%) showed extra-intestinal performance, all of which were joint swelling andpain. No skin manifestations, oral ulcer, anal fistula or eye diseases were seen.1case of patient had intestinal perforation and was undergone surgical treatment.Most of the UC patients were chronic recurrence type, accounting for51.3%,while initial onset type, chronic relapsing type and acute fulminant type were41.5%,6.5%and0.7%.Among the area of the lesions,33.0%were colitis of the whole colon,24.5%were colitis of the left hemicolon,22.9%were proctosigmoiditis and19.6%wereproctitis.62.4%of UC patients were mainly performed as multiple erosion andshallow ulcer under endoscope,30.7%as pseudo polyps and6.9%as stenosislumen. Analysis on UC pathological observation results showed that74.6%of thepatients were with mild chronic inflammation in mucosal tissue,21.0%withsevere chronic inflammation in mucosal tissue and inflammatory granulationtissue,4.3%with adenomatous hyperplasia and5.1%with microabscess in mucosarecess.3.Clinical characteristics of CDAmong the374cases,68cases were with Crohn’s Disease (CD). The meanaverage age of the UC patients (male-female ration of1.1:1) was46years, and thesmokers accounted for26.47%.The main manifestations of CD patients in digestive system were abdominalpain (82.3%), followed by diarrhea (42.6%) and abdominal mass (23.5%), andsome cases had hematochezia (23.5%).16.2%of the cases showed systemic feverwhile39.7%accompanied with weight loss.3cases (4.5%) showed extra-intestinalperformance, where one was with joint swelling and pain, the other two were withoral ulcer. No skin manifestations, anal fistula or eye diseases were seen.5cases had incomplete intestinal obstruction,2cases had intestinal perforation andabscess and the surgical history.Among the area of the lesions,23.6%(16/68) were limited to the terminalileum,57.4%(39/68) were colon,11.8%(8/68) were ileocolon and7.4%(5/68) ofthe lesions were involved in the area above ileum. CD patients were mainlyperformed as segmental lesion under endoscope. Polypoid apophysis andpaving-stone lesions were seen in25.3%of the patients, unregular ulcer anderosion seen in41.2%of the patients, luminal stenosis was seen in23.8%of thepatients, and congested and edematous mucous membrane was seen in9.5%of thepatients. Pathological performance of CD patients included chronic inflammatorycell infiltration of lamina propria, lymphocytic aggregation at the bottom of thecrypt and submucosa, submucosa broadening, fissuring ulcers and non-caseousgranulomas, etc.Conclusion The number of IBD inpatients admitted in General Hospital ofNingxia Medical University was significantly increased in the recent10years. Theclinical classification was given priority to chronic recurrence type, andextra-intestine performance was rare. Annual number of CD inpatients wasrelatively stable. No obvious rising trend was seen.
