Objective To explore the value of diffusion tensor imaging(DTI) and diffusion tensortractography(DTT)technique in severity assessment and prognosis prediction of diffuseanonal injury(DAI).Methords The6cases of DAI patients met the criteria were included DAI group.According to Glasgow Outcome Scale (GOS), the DAI group was subdivided into goodprognosis group(2cases) and poor prognosis group(4cases,including2deaths) after3months.Glasgow coma scale(GCS) was used to assess the consciousness of DAI patients atadmission. All the patients received MR inspection the data of DTI within three days afteronse.DTT can visualize the major white matter tract by setting ROI on the path of the fiberson the basic of DTI.The fractional anisotropy(FA) values in different regions ofinterest(ROI)were measured and compared. This study selected brain stem(midbrain, pons andmedulla oblongata) as regions of interest. Results Six patients with DAI by positive normalized comprehensive treatment of2cases good recovery,4cases of patients with poor prognosis. The ROI area FA values existcorresponding relation with the severity of the clinical manifestation and prognosis ofrecovery.Tend to FA value is smaller, the clinical manifestations of the patients with the weightand the relative to the worse prognosis.DTT technique shows that patients with clinicalmanifestations are more serious if axonal fiber bundle are more intricate and even interrupt. Conclusions DTI and DTT technology can intuitive reflect axonal injury, has acertain value in judge injury and prognosis evaluation with DAI.
