Development And Its Clinical Application Of A Temperature Controllable Device For Chinese Herbal Fumigation For Treatment Of Proximal Humeral Fractures

Objective:To develop a temperature controllable device for Chinese herbal fumigation,and investigate its effectiveness in the rehabilitation after proximal humeral fractures in order to provide clinical evidence.Methods:To develop a temperature controllable device for Chinese herbal fumigation. This topic selected studies 59 patients, were collected from July 2013 to November 2014 in line with proximal humeral fractures (Neer classification:NeerⅡ、Ⅲ、Ⅳ type fractures) of our hospital, including 35 males and 24 females with an average age of 51.36 years (range,23-58 years). All the patients were divided randomly into control group(n=20), experimental group A(n=19)and experimental group B(n=20), according to the order of admission. The treatments as followed:Basic treatment:all the patients were treated with plaster external fixation as well as drug treatment for relieve pain after admission, operation was carried within 5 days after admission. Pain relief, forearm sling and prescription of antibiotics were given postoperatively. Drainage tubes were removed in 2-3 days postoperatively;functional exercise was applied as following treatment. Intervention treatment:After the wound healed well 2 weeks later, all the patients taken functional exercise sequentially. In experimental group A, patients were additionally given traditional fumigation-washing. In experimental group B, patients were additionally given temperature controllable device for Chinese herbal fumigation.Observation index:①Pain intensity (VAS score);② Daily life score;③ Shoulder Joint range of motion; ④ The shoulder joint function assessment:using the Constant-Murley shoulder score standard. Observe and record the three groups of patients with postoperative for second week, fourth week, sixth week after evaluation of the results, and access to data. All data of thre groups were calculated by SPSS 18.0 for windows for statistic analysis.Results:(1)There was no statistical significance in general data between three groups of patients(P>0.05). (2) Comparison in the groups,all the observation indexes of each group after 2 weeks,4 weeks,6 weeks were statistically significant(P<0.05). Comparison among the groups, there were no statistical significance of all the observation indexes between three groups on the 2nd week postoperatively(P>0.05);all the observation indexes besides daily life score of experimental group A and experimental group B had statistical significance compared with the control group on the 4th week after operation(P<0.05); all the observation indexes of experimental group A and experimental group B has statistical significance compared with the control group on the 6th week postoperatively(P<0.05);there has statistical significance of the before lifting ROM between experimental group A and experimental group B on the 4th week postoperatively(P<0.05); all the observation indexes of experimental group B has statistical significance compared with experimental group A on the 6th week after operation(P<0.05).Conclusions:Temperature controllable device for Chinese herbal fumigation have better clinical recovery on Pain intensity, Daily life score,ROM and Assessment of shoulder function than traditional fumigation-washing for treatment of proximal humeral fractures.
