Objective:By observing the clinical curative effect of Xinhuang pills combined with low molecular heparin sodium for the prevention of deep vein thrombosis after the around hip fractures, to accumulate the experience of traditional Chinese medicine preventing the lower limb deep vein thrombosis.Methods:Seventy-four patients within inclusion criteria and exclusion criteria between December 2013 to December 2014 from the 175th hospital of People Liberation Army were elected. According to the admission time successively, patients were randomly divided into treatment group (n=37) and control group (n=37). control group was injected low molecular weight hepar sodium in subcutan eously 100 IU/kg once daily, and treatment group were treated with Xinhuang pills additionally (0.64g twice a day). Intraoperative blood loss, postoperative total flow and operation mode were recorded. Respectively admission day, after 3, 7,12 days of operation blood clotting function, D-dimer, blood platelet count,double lower limbs deep vein color doppler ultrasound examination were taken. Week of limb diameter and pain relief were measured.The data were analyzed using statistical software SPSS17.0, setting P <0.05 had statistical significance.Results:1.There were three patients had thrombosis in treatment group, eleven patients in control group. In treatment group, it was respectively found one case, two cases, in admission day and after 3 days of operation,one case in femoral vein, two cases in popliteal vein. In control group, it was respectively found three case,four cases, four cases, in admission day, and after 3,7 days of operation, six cases in femoral vein, five cases in popliteal vein. The incidence of thrombosis were respectively 8.11%,29.73%. The incidence of thrombosis between the two groups had statistical significance (P<0.05).2.The differences of PT, APTT, FIB and D-dimer between the groups had no statistical significance (P>0.05) in admission day. In after 3,7,12days of operation the differences of PT, APTT, FIB and D-dimer between the groups were statistically significant (P<0.05). The INR was similar between the two groups had no statistical significance (P>0.05).3.The differences of PLT between the groups had no statistical significance (P>0.05) in admission day. after 3,7,12 days of operation the differences of PLT between the groups were statistically significant (P<0.05).4. The difference of intraoperative blood loss and postoperative total flow between the groups had no statistical significance (P>0.05).5. After 3,7,12 days of operation, the differences of limb and the contralateral limb weeks diameter difference and pain score between the groups were statistically significant (P<0.05).Conclusion:The Xinhuang pills combined with low molecular heparin sodium can relieve local swelling and pain, and improve local blood circulation and improve the blood coagulation state and the blood fibrinolytic activity. It has good prevention effect for the deep vein thrombosis after the around hip fracture. And preoperative application of the Xinhuang pills do not increase the risk of bleeding, reflecting the safty of the medication. It shows that the Xinhuang pills can be early applied to prevent the deep vein thrombosis after after the around hip fracture. The further work can study the mechanisms through animal experiments.
